| dc.description.abstract | การศึกษาสถานการณ์การดูแลรักษาผู้ป่วยภาวะไตล้มเหลวเรื้อรังที่เข้ารับการรักษาในโรงพยาบาล โดยใช้ฐานข้อมูลผู้ป่วยในของสำนักงานหลักประกันสุขภาพแห่งชาติ ปีงบประมาณ 2548 เลือกผู้ป่วยที่มีรหัสการวินิจฉัยเป็นภาวะไตล้มเหลวเรื้อรัง โดยใช้สถิติเชิงพรรณนาวิเคราะห์ข้อมูล ได้ผู้ป่วย 45,326 ราย เข้ารับการรักษา 83,734 ครั้ง ในโรงพยาบาล 929 แห่ง คิดเป็นค่าเฉลี่ย 1.84 ครั้งต่อผู้ป่วย 1 ราย, จำนวนวันเฉลี่ย 5.4 วัน ผู้ป่วยมีอายุเฉลี่ย 63.9 ปี, ร้อยละ 56.19 เป็นผู้หญิง ผู้ป่วยกว่า 1 ใน 3 เข้ารับการรักษามากกว่า 1 ครั้งในรอบปี ผู้ป่วยส่วนใหญ่เข้ารับการรักษาโรคที่เกี่ยวข้องกับภาวะมีสารน้ำคั่งอยู่ในร่างกายมากเกินไป และจากสาเหตุการอักเสบติดเชื้อ ผู้ป่วยร้อยละ 95 เป็นหลายโรค, ครึ่งหนึ่งมี 3-4 โรค การให้เลือดเป็นวิธีการรักษาที่ให้บ่อยที่สุด ผู้ป่วยจำนวนหนึ่งได้รับการฟอกเลือดและล้างไต ซึ่งเป็นกรณีที่ไม่ได้รับความคุ้มครองตามสิทธิประโยชน์ ค่าใช้จ่ายที่ดูแลรักษาผู้ป่วยโรคไตล้มเหลวเรื้อรังในโรงพยาบาลทั้งหมด 917 ล้านบาท ค่ามัธยฐาน 4,845 ราย อย่างไรก็ตาม ความถูกต้องของข้อมูลเป็นข้อจำกัดของฐานข้อมูลผู้ป่วยที่สำคัญ ข้อมูลที่มีปัญหามากที่สุด คือ รหัสการวินิจฉัยโรค, รหัสหัตถการ, จำนวนเงินที่โรงพยาบาลเรียกเก็บ, และสถานภาพของผู้ป่วยเมื่อจำหน่าย ข้อมูลเกี่ยวกับลักษณะทั่วไปของผู้ป่วยมีความถูกต้องค่อนข้างสูง | th_TH |

| dc.description.abstractalternative | The objective of this study was to assess the situation of in-patient care for chronic renal failure patients under the Universal Coverage Scheme. The in-patient claim database for fiscal year 2005, compiled by the Central Office for Health-care Information, was used for the analysis. All admissions with ICD-10 codes compatible with chronic renal failure were included. There were 45,326 chronic renal failure patients who accounted for 83,734 admissions (1.84 admissions per patient) in 929 hospitals. The average length of stay was 5.4 days. Fifty-six percent of patients were female, with the average age being 63.9 years. One-third of the patients had multiple admissions and 95 percent had multiple morbidities. The most common diagnoses were related to hypervolemic problems or infections. Blood transfusions were the most common treatment procedure . Hemodialysis and peritoneal dialysis were performed on a number of the admissions even though they were not covered by the benefit package. Total hospital charges were 917 million baht (median 4,845 baht per admission). Approximately 82 percent of the patients were discharged with clinical improvement; the fatality rate was 9.84 percent (4,462 deaths). The validity of data was the major limitation regarding diagnostic and procedural coding, hospital charges, and discharge status. The patient demographic information was quite valid. | en_US |
