| dc.description.abstract | การศึกษาผู้ป่วยโรคเก๊าต์ที่มีภาวะไตเสื่อมและการใช้ยารักษาผู้ป่วยที่โรงพยาบาลหนองจิก จังหวัดปัตตานี รวม 122 ราย เพื่อนำผลข้อมูลไปประยุกต์ใช้ในด้านความปลอดภัยของการรักษา โดยวิเคราะห์จากฐานข้อมูลในโปรแกรมบริการของโรงพยาบาลหนองจิก ในช่วงเดือนตุลาคม 2550- เดือนธันวาคม 2552 โดนคำนวณหาค่าประมาณอัตราการกรองผ่านไต และเสนอข้อมูลเป็นจำนวน, ค่าร้อยละ, ค่าเฉลี่ย และค่าเบี่ยงเบนมาตรฐาน และโดยการวิเคราะห์แบบถดถอยพหุลอจิสติก เพื่อแสดงความสัมพันธ์ระหว่างปัจจัยต่างๆ กับอัตราการกรองผ่านไตที่ต่ำกว่า 60 มล./นาที/1.73 ตร.ม. จากการศึกษาพบผู้ป่วยเพศชายมากกว่าเพศหญิงประมาณ 2.7 เท่า อายุเฉลี่ย 64 ปี และมักมีโรคอื่นร่วมด้วย ผู้ป่วยส่วนใหญ่ได้รับยาแอลโลพูรินอลควบคู่กับคอลชิศีน และพบการใช้ยาคอลชิศีลในผู้ป่วยโรคไตเรื้อรังระยะที่4 และ5 อย่างต่อเนื่องโดยไม่ได้ปรับขนาดยา ผู้ป่วยร้อยละ 58.2 มีค่าประมาณอัตราการกรองผ่านไตต่ำกว่า 60 มล./นาที/1.73 ตร.ม. โดยผู้ป่วยร้อยละ 81.7 เป็นโรคความดันโลหิตสูงร่วมด้วย และพบว่าอายุเป็นปัจจัยที่มีความสัมพันธ์อย่างมีนัยสำคัญทางสถิติกับอัตราการกรองผ่านไตที่ต่ำกว่า 60 มล./นาที/1.73 ตร.ม. การคัดกรองภาวการณ์ทำงานของไตในผู้ป่วยโรคเก๊าต์โดยคำนวณหาค่าประมาณอัตราการกรองผ่านไตก่อนผู้ป่วยพบแพทย์เป็นสิ่งสำคัญสำหรับโรงพยาบาลชุมชน ซึ่งจะทำให้มีตรวจพบภาวะไตเสื่อมในโรคเก๊าต์ตั้งแต่ระยะต้นๆและช่วยให้มีการปรับยาให้เหมาะสมกับการทำงานของไต ซึ่งจะทำให้เกิดความปลอดภัยต่อการใช้ยา ป้องกันการเสื่อมของไตเพื่อไม่ให้ดำเนินไปสู่ภาวะไตล้มเหลวระยะสุดท้าย หรือชะลอให้นานที่สุด | en_US |

| dc.description.abstractalternative | This descriptive research was conducted to determine the prevalence of renal impairment and drug use in gout patients treated at Nongjik Hospital, Pattani Province. The methodologies included a review of the medical records of 122 patients in the period from October 2008 to December 2009; estimated glomerular filtration rate (eGFR) was calculated by the MDRD equation. Data analysis provided the frequency, percentage, mean and standard deviation. Chi-square test was applied to compare between proportions and multiple logistic regression was used to determine associations between the prevalence of eGFR < 60 ml/min/1.73 m2. The majority of patients were male, with their average age being 64 years. Most patients had comorbid diseases. Medications used were concurrent allopurinol and colchicine. A significant proportion of patients had chronic kidney disease stage 4 and 5 which was treated with colchicine without dose adjustment. Of the total, 58.2 per cent of the patients exhibited eGFR <60 ml/min/1.73 m2 and 81.7 per cent of them showed hypertension. Age was the only significant factor associated with eGFR < 60 ml/ min/1.73 m2 as revealed by multiple logistic regression analysis. Based on the experience obtained in this study, renal function testing for gout patients is important for early detection of renal impairment; the finding would also be useful for dose adjustment of the drugs prescribed, based on which end stage renal disease may be prevented or slowed down. | en_US |
