| dc.description.abstract | เป็นการศึกษาความพร้อมในการทำงานระบบเครือข่ายรวมทั้งศึกษาปัจจัยที่ส่งเสริมและปัจจัยที่เป็นอุปสรรคของสถานบริการสาธารณสุขระดับปฐมภูมิ 3 แห่งในเขตอำเภอเมืองจังหวัดนครปฐม โดยใช้การสัมภาษณกึ่งโครงสร้างและการสังเกตการณ์แบบไม่มีส่วนร่วม พบว่าแต่ละแห่งมีระดับความพร้อมในการทำงานที่แตกต่างกัน ศูนย์แพทย์ชุมชนองค์ประปฐมเจดีย์มีความพร้อมมากที่สุด รองลงมาคือสถานีอนามัยสนามจันทร์ และที่ยังไม่มีความพร้อมคือ ศูนย์บริการสาธารณสุขเทศบาลนครปฐม ปัจจัยที่ส่งเสริมให้เกิดความพร้อมในการทำงานระดับเครือข่าย คือ ลักษณะของชุมชนทางกายภาพ ขนาดและระบบงบประมาณ ลักษณะการทำงานของบุคลากร ลักษณะการทำงานของผู้บริหาร กฎระเบียบขององค์การ ความรู้ในงานที่รับผิดชอบ ความเข้าใจในการทำงานระบบเครือข่ายของผู้บริหาร การจัดสรรอัตรากำลัง กระบวนการประกันและการควบคุมคุณภาพการปฏิบัติงานของเครือข่าย การดำเนินงานร่วมกับหน่วยงานประเภทอื่น และระบบการติดตามประเมินผลที่มีประสิทธิภาพ ส่วนปัจจัยที่เป็นอุปสรรคของความพร้อมในการทำงานระบบเครือข่าย คือขาดนโยบายที่เน้นให้หน่วยงานที่มีส่วนได้ส่วนเสียเข้ามามีส่วนร่วม ไม่มีการใช้เทคโนโลยีสารสนเทศเป็นเครื่องมือในการประสานงาน ไม่มีการวางแผนทิศทางที่มุ่งเน้นผลงาน มีโครงสร้างขององค์การและสายการบังคับบัญชาที่ทำให้การทำงานมีหลายขั้นตอน ขาดการบริหารที่สร้างขวัญและกำลังใจแก่ผู้ปฏิบัติงาน ไม่มีความครบถ้วนของผู้มีส่วนได้ส่วนเสียที่เข้ามาเป็นเครือข่าย ไม่มีองค์ความรู้ที่สร้างใหม่จากกระบวนการบริหารจัดการความรู้ และไม่มีระบบการบริหารต้นทุนหรือค่าใช้จ่ายอย่างมีประสิทธิภาพ | en_US |

| dc.description.abstractalternative | The organizations concerned with primary care have tried to develop the effectiveness of their service for all people equally. This research has as its main objectives studying the readiness of networks for network operations and studying supporting factors and obstacles found in three health centers, namely Phra Pathom Chedi Community Medical Center, Sanam Chan Sanitarium and Nakhon Pathom Municipality Health Center. The methodology utilized included semi-structured interview and participatory observation. The results of the study revealed that all three centers had different levels of readiness for network operations. Phra Pathom Chedi Community Medical Center had comparatively the highest degree of readiness. The second was Sanam Chan Sanitarium whereas Nakhon Pathom Municipality Health Center did not show readiness. The factors affecting the readiness in network operations were community characteristics, size, budget administration system, staff working style, executive working style, organization regulations, knowledge and capability in responsible jobs, executives’ roles in network operations, manpower allocation, quality assurance and control of network operations, cooperation with other organizations, and monitoring and evaluation systems. It was also found that the obstacles to networking include lack of policy for stakeholders’ participation, no information technology for coordination, no performancebased direction planning, complex organizational structure and line of command, inadequate morale of staff, insufficient stakeholders in the network, no knowledge management or learning organization practice, and no effective unit cost management system. | en_US |
