In vitro sensitivity and resistance mechanism of artemisinin derivatives and their combined antimalarial drugs in Plasmodium falciparum

dc.description.abstractการศึกษาครั้งนี้มีวัตถุประสงค์เพื่อศึกษาความไวต่อยาต้านมาลาเรียกลุ่มอนุพันธ์ artemisinin และยาที่ใช้ร่วมใน artemisinin derivative based combination therapy (ACT) และลักษณะทางพันธุกรรมของการดื้อยาในเชื้อ Plasmodium falciparum จากชายแดนไทยพม่าและไทยกัมพูชาจำนวน 95 สายพันธุ์ พบว่าเชื้อเกือบทั้งหมดเป็นเชื้อที่ไม่ดื้อต่อ artesunate และ dihydroartemisinin พบจุดกลายพันธุ์บนยีนดื้อยา artemisinin, Kelch13 propeller ประมาณ 77% ซึ่งจุดกลายพันธุ์ส่วนใหญ่คือ C580Y เชื้อที่แยกได้จากชายแดนไทยกัมพูชามี pfmdr1 184F allele ทุกสายพันธุ์และ copy number ประมาณ 1 copy เชื้อที่แยกได้จากชายแดนไทยพม่าพบว่ามี pfmdr1 184F allele อยู่ 47.2% และมี copy number เฉลี่ย 2.5 copy เชื้อส่วนใหญ่ดื้อต่อ mefloquine ซึ่งอาจนำมาสู่การล้มเหลวจากการรักษาโดย artesunate-mefloquine ในประเทศไทย กระทรวงสาธารณสุขจึงได้พิจารณาเปลี่ยนยาหลักสำหรับการรักษามาลาเรียที่เกิดจากเชื้อ P. falciparum เป็น dihyroartemisinin-piperaquine การศึกษาเชื้อ P. falciparum ในครั้งนี้พบว่าเชื้อที่มี pfmdr1 86Y allele จะมีความไวต่อ piperaquine ลดลง การนำ dihydroartemisinin-piperaquine มาใช้ในประเทศไทย ควรมีการทดสอบก่อนใช้และการเฝ้าระวังอย่างเหมาะสมโดยเฉพาะอย่างยิ่งชายแดนไทยมาเลเซีย ซึ่งเชื้อส่วนใหญ่มี pfmdr1 86Y allele artesunate-pyronaridine เป็น ACT ใหม่ที่มีประสิทธิภาพ ค่า pyronaridine IC50 ของเชื้อในการศึกษาครั้งนี้ต่ำกว่าค่าที่เคยรายงานไว้ในเชื้อ recrudescence ในประเทศไทย 3-220 เท่า เมื่อเปรียบเทียบเชื้อ P. falciparum ที่มี pfmdr1 haplotype ต่างกัน ไม่พบว่าค่า IC50 ของ pyronaridine มีความแตกต่างกันในแต่ละกลุ่ม ดังนั้น artesunate-pyronaridine อาจเป็นยา ACT ที่เหมาะสมสำหรับพื่นที่ ที่เชื้อ P. falciparum มีความหลายหลายของลักษณะยีนดื้อยาโดยเฉพาะอย่างยิ่งยีน pfmdr1 ลักษณะทางพันธุกรรมของยีนดื้อยาที่แตกต่างกันทำให้เชื้อ P. falciparum มีความไวต่อยาต้านมาลาเรียชนิดต่างๆ ที่แตกต่างกัน การทดสอบความไวต่อยาต้านมาลาเรียชนิดต่างๆ และการสำรวจข้อมูลพื้นฐานของลักษณะทางพันธุกรรมของยีนดื้อยาในเชื้อ P. falciparum ที่แยกได้จากพื้นที่ต่างๆ ของประเทศไทยควรจะต้องทำอย่างต่อเนื่อง เพื่อเป็นการเฝ้าระวังการพัฒนาการดื้อยาของเชื้อ ข้อมูลเหล่านี้ยังนำไปใช้พิจารณาการใช้ยาอย่างเหมาะสมth_TH

dc.description.abstractalternativeThis study aimed to determine in vitro sensitivities of artemisinin derivatives and their partner drugs in artemisinin derivative based combination therapy (ACT) and genetic characterization of resistant genes in 95 Plasmodium falciparum isolates collected from Thai-Myanmar and Thai-Cambodia borders. Almost all isolates were sensitive to artesunate และ dihydroartemisinin. Approximately seventy seven percent of the samples had a mutation on an artemisinin-resistant gene, Kelch13 propeller which C580Y mutation was predominant. All parasites isolated from Thai-Cambodia border contained 184F allele with 1 copy number of pfmdr1 gene. In contrast, the parasites collected from Thai-Myanmar border had the pfmdr1 184F allele at 47.2% and contained a few copy number of the pfmdr1 gene with the average of 2.5 copies. Most isolates were resistant to mefloquine which might lead to treatment failure of artesunate-mefloquine in Thailand. Consequently, The Ministry of Public Health has changed the first-line treatment for falciparum malaria to dihyroartemisinin-piperaquine. The present study shows that the parasites containing the pfmdr1 86Y allele exhibited reduced piperaquine sensitivity. Prior to nationwide implication of dihydroartemisinin-piperaquine usage in Thailand, efficacy testing should be conducted. In addition, regular monitoring should be properly performed especially in Thai-Malaysia border where most parasites contained the pfmdr1 86Y allele. Artesunate-pyronaridine is a new effective ACT. The parasites’ pyronaridine IC50 in this study were 3-220 times lower than the IC50 of a recrudescent parasite previously reported in Thailand. In addition, there was no significant difference of pyronaridine IC50 among the parasites with different haplotypes. Thus, artesunate-pyronaridine may be a suitable ACT of choice in the area containing parasites with varied resistant haplotypes especially the pfmdr1 haplotypes. In vitro sensitivities and genotypic characterization of the resistant genes of P. falciparum should be continuously monitored in order to detect the emergence of antimalarial resistance. Rationale drug use can be guided using this information.en_EN
