dc.description.abstractในปี 2547 กระทรวงสาธารณสุขได้มีการประกาศขายบัตรประกันสุขภาพให้คนต่างด้าวที่ไม่มีสิทธิในหลักประกันสุขภาพใดๆ โดยคนต่างด้าวที่ผ่านการตรวจสุขภาพจะได้รับอนุญาตให้ทำงานและซื้อบัตรประกันสุขภาพของกระทรวงสาธารณสุข ทั้งนี้ รายการตรวจสุขภาพและสิทธิประโยชน์ต่างๆ ภายใต้บัตรประกันสุขภาพฯ ดังกล่าวนั้น มีการเปลี่ยนแปลงไม่มากนักในช่วงเวลา 10 ปีที่ผ่านมา ดังนั้น การศึกษานี้จึงมีวัตถุประสงค์เพื่อจัดลำดับความสำคัญของภาวะสุขภาพและโรคไม่ติดต่อสำคัญของคนต่างด้าวเพื่อนำไปใช้ในการพิจารณาปรับปรุงรายการการตรวจสุขภาพและสิทธิประโยชน์ของคนต่างด้าวให้สอดคล้องกับสถานการณ์ปัจจุบัน โดยคณะผู้วิจัยได้ทบทวนข้อมูลภาวะสุขภาพและโรคไม่ติดต่อของคนต่างด้าวจากเอกสารต่างๆ ได้แก่ ภาระโรค รายงานการตรวจคัดกรองสุขภาพจากศูนย์บริการเบ็ดเสร็จและการศึกษาต่างๆ ที่เกี่ยวข้อง นอกจากนี้ คณะผู้วิจัยยังได้วิเคราะห์ฐานข้อมูลการเบิกจ่ายภายใต้บัตรประกันสุขภาพฯ เพื่อค้นหาภาวะสุขภาพและโรคไม่ติดต่อที่คนต่างด้าวมารับบริการและมีการเบิกจ่ายมาก จากนั้นคณะผู้วิจัยได้นำเสนอผลจากการทบทวนเอกสารที่เกี่ยวข้องและการวิเคราะห์ฐานข้อมูลดังกล่าวในการประชุมผู้มีส่วนได้ส่วนเสีย เพื่อจัดลำดับความสำคัญของภาวะสุขภาพและโรคไม่ติดต่อ โดยใช้วิธีเดลฟายมาประยุกต์ ผลการจัดลำดับความสำคัญของภาวะสุขภาพและโรคไม่ติดต่อ 10 รายการ เรียงจากความสำคัญมากไปน้อย คือ ภาวะการตั้งครรภ์ เบาหวาน ความผิดปกติทารกแรกเกิด ความดันโลหิตสูง การติดสารเสพติดให้โทษและพิษสุราเรื้อรัง การบาดเจ็บจากการทำงาน การบาดเจ็บที่ศีรษะ ความผิดปกติทางจิต มะเร็งเต้านมและมะเร็งปากมดลูก ตามลำดับth_TH

dc.description.abstractalternativeIn 2004, the Ministry of Public Health (MoPH) introduced the nationwide Health Insurance Card Scheme (HICS) for migrants who were not covered by the Social Security Scheme (SSS). Before being insured and granted work permits, these migrants must pass the health screening procedures first. However, the health screening and benefits package of the HICS have not been changed much throughout the last 10 years. The present study was conducted with an aim to revise the health screening guideline and benefit package in accordance with current disease situation by prioritization study on health conditions among migrants in Thailand. The health conditions and non-communicable diseases were reviewed. The document sources were received from the burden of disease study, and the health screening report of the One Stop Service. Another related source was the reimbursement database of HICS for migrants on the frequency, mean, and total reimbursement of high-cost care. By using modified Delphi consensus process, the research team presented the result of documentary reviews and database analysis to stakeholder meeting before prioritization process. The top 10 health conditions and non-communicable diseases in the final round consisted of pregnancy, diabetes, neonatal abnormalities, high blood pressure, substance abuse and alcoholism, work injury, head injury, mental disorders, breast cancer and cervical cancer respectively.en_EN
