| dc.description.abstract | โครงการนี้ดำเนินการตามข้อเสนอคณะทำงานขับเคลื่อนความเพียงพอและความยั่งยืนของงบประมาณ เป็นการปรับปรุงการจัดทำบัญชีต้นทุนสถานพยาบาล เป็นมาตรการ Prerequisite ระยะสั้นของคณะทำงานขับเคลื่อนความเพียงพอและความยั่งยืนของงบประมาณภายใต้คณะกรรมการปฏิรูปประเทศชุดที่ 1 ที่มีเป้าหมายเพื่อให้เกิดกระบวนการนำไปใช้เป็นรูปแบบการรายงานข้อมูลต้นทุนของสถานพยาบาลที่กำหนดหรือปฏิบัติ เป็นแนวทางเดียวกันทั้งประเทศได้ การวิจัยนี้เป็นการศึกษาเชิงคุณภาพโดยเป็นการทดลองใช้รายงานต้นทุนบริการในพื้นที่นำร่อง เพื่อทดสอบระบบ Platform ปัญหาและอุปสรรค ตลอดจนความเข้าใจในการนำเข้าข้อมูลและการวิเคราะห์ข้อมูล ตลอดจนการนำไปใช้ในพื้นที่นำร่อง รวมถึงปรับปรุงระบบให้สมบูรณ์ก่อนการพัฒนาเพื่อนำไปขยายผลใช้ในระดับประเทศ โดยมีการดำเนินงานประกอบด้วย 4 ส่วน คือ ส่วนที่ 1. จัดทำ Platform ระบบฐานข้อมูลและคู่มือมาตรฐานรายงานต้นทุน (Standard Cost Report) ของสถานพยาบาล ส่วนที่ 2. ประสานงานสถานพยาบาลผ่านผู้ตรวจราชการของเขตสุขภาพในพื้นที่นำร่องและจัดการอบรมเชิงปฏิบัติการใช้ Platform และการเข้าถึงฐานข้อมูลต้นทุนบริการจำแนกตามประเภทสถานพยาบาลที่ผู้เข้าร่วมอบรมสังกัด ส่วนที่ 3. สัมภาษณ์เชิงลึกและ/หรือสนทนากลุ่ม ภายหลังจากการจัดการทดลองใช้ Platform และส่วนที่ 4. การจัดทำฐานข้อมูลให้สอดคล้องกับทุกประเภทสถานพยาบาล ผลการวิจัย พบว่า กระบวนการขยายไปสู่การปฏิบัติ สามารถกำหนดให้ทุกสถานพยาบาลในภาครัฐได้ โดยมีลำดับการจัดทำ คือ โรงพยาบาล (รพ.) ที่มีความพร้อมสามารถดำเนินการได้เป็นลำดับแรก คือ รพ. สังกัดสำนักงานปลัดกระทรวงสาธารณสุข พร้อมกับ รพ. สังกัดอื่นนอกสำนักงานปลัดกระทรวงสาธารณสุขแต่อยู่ภายใต้กระทรวงสาธารณสุข แล้วจึงขยายไปสู่ รพ. นอกสังกัดกระทรวงสาธารณสุข ในขณะที่ รพ. เอกชน จำเป็นต้องใช้การบังคับใช้ผ่านกฎหมาย สำหรับระบบฐานข้อมูลมีระบบการนำเข้าข้อมูล (Import Data) โดยต้นสังกัดสามารถนำเข้าข้อมูลจากฐานข้อมูลรายงานทางการเงินของทุก รพ. เข้ามาในระบบได้ อย่างไรก็ดีระบบฐานข้อมูลยังมีข้อจำกัดที่ต้องมีรายงานทางการเงินที่อยู่ในรูปแบบเดียวกัน หรือมีผังบัญชีย่อยที่เหมือนกันดังนั้นการนำเข้าข้อมูลจำเป็นต้องแปลงให้อยู่ในคำจำกัดความและระบบข้อมูลเดียวกันก่อนจึงทำการนำเข้าข้อมูล | th_TH |

It is an improvement in hospital cost accounting. In-depth interviews and/or group discussions After organizing a trial of the platform and Part 4. Creating a database to be consistent with all types of medical facilities. The research results found that the process extends into practice. Can be assigned to every hospital in the government sector immediately. The order of preparation is that hospitals can proceed immediately with hospitals under the Office of the Permanent Secretary, Ministry of Public Health, along with other hospitals outside the Office of the Permanent Secretary of the Ministry of Public Health but under the Ministry of Public Health, then expand to hospitals outside the Ministry of Public Health, while private hospital enforcement through law is required. For the database system, there is a data import system (Import Data). The agency can import data from the financial reporting database of every hospital into the system.
