dc.description.abstractการวิจัยครั้งนี้เป็นงานวิจัยเชิงวิเคราะห์แบบภาคตัดขวาง (Cross-sectional analytical study) ทำการเก็บข้อมูลทั้งเชิงปริมาณและเชิงคุณภาพ มีวัตถุประสงค์เพื่อ 1) อธิบายความชุกของความเสี่ยงโรคเบาหวานของแรงงานต่างด้าวชาวพม่าในจังหวัดสุราษฎร์ธานี 2) อธิบายความชุกของความเสี่ยงโรคความดันโลหิตสูงของแรงงานต่างด้าวชาวพม่าในจังหวัดสุราษฎร์ธานี 3) ประเมินปัจจัยที่มีความสัมพันธ์กับความเสี่ยงโรคเบาหวานของแรงงานต่างด้าวชาวพม่าในจังหวัดสุราษฎร์ธานี 4) ประเมินปัจจัยที่มีความสัมพันธ์กับความเสี่ยงโรคความดันโลหิตสูงของแรงงานต่างด้าวชาวพม่าในจังหวัดสุราษฎร์ธานี และ 5) หาแนวทางในการแก้ปัญหา รวมทั้งข้อเสนอแนะเชิงนโยบายในการพัฒนาสถานการณ์ความเสี่ยงของโรคเบาหวานและความเสี่ยงโรคความดันโลหิตสูงของแรงงานต่างด้าวชาวพม่าในจังหวัดสุราษฎร์ธานี กลุ่มตัวอย่าง คือ แรงงานต่างด้าวชาวพม่าที่มีการปฏิบัติงานอยู่ในจังหวัดสุราษฎร์ธานี จำนวน 508 คน การวิจัยในระยะที่ 1 เชิงปริมาณ ทำการเก็บข้อมูลด้วยการสัมภาษณ์ เครื่องมือที่ใช้ในการเก็บข้อมูลเป็นแบบสัมภาษณ์ที่ผู้วิจัยสร้างขึ้นเองตามกรอบแนวคิดและการทบทวนวรรณกรรมที่เกี่ยวข้อง โดยมีค่าสัมประสิทธิ์อัลฟาครอนบาคของเครื่องมือด้านความเครียด ด้านทัศนคติ และด้านการปฏิบัติตัว เท่ากับ 0.78, 0.87 และ 0.82 และด้านความรู้ มีค่าสัมประสิทธิ์คูเดอร์-ริชาร์ดสัน เท่ากับ 0.77 ทำการสุ่มแบบหลายขั้นตอน (Multi-stage random sampling) และทำการวิเคราะห์ข้อมูลเชิงอนุมานด้วยสถิติ Multiple logistic regression และการวิจัยในระยะที่ 2 เชิงคุณภาพ ทำการเก็บข้อมูลด้วยการสนทนากลุ่มและการประชุมสหสาขาวิชาชีพของภาคีเครือข่ายทางด้านสุขภาพและผู้มีส่วนได้ส่วนเสียที่เกี่ยวข้อง ผลการวิจัยพบว่า แรงงานดังกล่าวมีค่าระดับน้ำตาลในเลือดอยู่ในกลุ่มปกติ ร้อยละ 81.30 (95% CI: 77.66–84.46) มีค่าระดับน้ำตาลในเลือดอยู่ในกลุ่มเสี่ยง ร้อยละ 12.60 (95% CI: 9.97-15.78) และมีค่าระดับน้ำตาลในเลือดอยู่ในกลุ่มต้องส่งยืนยันการวินิจฉัยโดยแพทย์ ร้อยละ 6.10 (95% CI: 4.32-8.55) แรงงานดังกล่าวมีค่าความดันโลหิตอยู่ในกลุ่มปกติ ร้อยละ 31.70 (95% CI: 27.78-35.88) มีค่าความดันโลหิตอยู่ในกลุ่มเสี่ยง ร้อยละ 43.70 (95% CI: 39.43-48.06) มีค่าความดันโลหิตอยู่ในกลุ่มต้องส่งยืนยันการวินิจฉัยโดยแพทย์ ร้อยละ 22.24 (95% CI: 18.83-26.07) และมีค่าความดันโลหิตอยู่ในกลุ่มต้องส่งพบแพทย์ภายในวันเดียวกัน ร้อยละ 2.36 (95% CI: 1.34-15.78) ปัจจัยที่มีความสัมพันธ์กับความเสี่ยงโรคเบาหวาน ประกอบด้วย สำเร็จการศึกษาต่ำกว่ามัธยมศึกษาตอนต้น (Adj.OR=2.12; 95%CI: 1.09–4.10;p-value=0.026) สถานภาพคู่ (Adj.OR=1.96; 95%CI: 1.04–3.70; p-value=0.037) การไม่ได้รับสนับสนุนสื่อหรือวัสดุต่างๆ ในรูปแบบภาษาพม่า (Adj.OR=1.78; 95%CI: 1.11–2.85; p-value=0.016) และการมีปัญหาสภาวะแวดล้อมในที่อยู่อาศัยในระดับปานกลาง/มาก (Adj.OR=1.71; 95%CI: 1.05–2.77; p-value=0.028) และปัจจัยที่มีความสัมพันธ์กับความเสี่ยงโรคความดันโลหิตสูง ประกอบด้วย อายุ >30 ปี (Adj.OR=2.77; 95%CI: 1.77–4.33; p-value<0.001) ดัชนีมวลกายในระดับน้ำหนักเกิน/อ้วน (Adj.OR=2.30; 95%CI: 1.50–3.53; p-value<0.001) รายจ่ายเฉลี่ยต่อเดือนทั้งครอบครัว ≥ 5,000 บาท (Adj.OR=2.28; 95%CI: 1.17–4.44; p-value=0.015) การไม่ได้รับข้อมูลข่าวสารทางด้านสุขภาพ (Adj.OR=2.28; 95%CI: 1.44–3.61; p-value<0.001) เพศชาย (Adj.OR=1.97; 95%CI: 1.27–3.07; p-value= 0.002) มีระยะเวลาที่ทำงานมาแล้ว ≥48 เดือน (Adj.OR=1.69; 95%CI: 1.06–2.71; p-value= 0.027) และจำนวนชั่วโมงในการทำงานต่อวัน ≤8 ชั่วโมง (Adj.OR=1.60; 95%CI: 1.03–2.49; p-value=0.036) ตามลำดับ โดยสรุปผลการศึกษานี้พบว่า แรงงานต่างด้าวชาวพม่าเกือบ 1 ใน 5 มีความเสี่ยงโรคเบาหวาน และ 2 ใน 3 มีความเสี่ยงโรคความดันโลหิตสูง ทั้งนี้ปัจจัยที่มีความสัมพันธ์กับความเสี่ยงโรคเบาหวานและความเสี่ยงโรคความดันโลหิตสูง ประกอบด้วย ด้านคุณลักษณะส่วนบุคคลและเศรษฐกิจและสังคม ด้านสิ่งแวดล้อม และด้านระบบบริการสุขภาพ ปัจจัยดังกล่าวนับว่าเป็นปัจจัยที่มีผลอย่างยิ่งต่อความเสี่ยงโรคเบาหวานและความเสี่ยงโรคความดันโลหิตสูง ข้อเสนอแนะเชิงนโยบายประกอบด้วย 1) การพัฒนาระบบการสร้างอาสาสมัครแรงงานต่างด้าวถือเป็นระบบที่มีความสำคัญอย่างยิ่งในการที่จะช่วยให้แรงงานสามารถเข้าถึงระบบบริการสาธารณสุขได้อย่างครอบคลุมมากยิ่งขึ้น 2) การบูรณาการภาคีเครือข่ายทางด้านสุขภาพทุกภาคส่วนในพื้นที่จังหวัดสุราษฎร์ธานี และมีการจัดตั้งเป็นคณะกรรมการส่งเสริมสุขภาพแรงงานต่างด้าวในระดับจังหวัด ถือเป็นกระบวนการที่มีประโยชน์ยิ่งในการบูรณาการทรัพยากรร่วมกันเพื่อการพัฒนาระบบการดูแลสุขภาพของแรงงานth_TH

dc.description.abstractalternativeThis research was a cross-sectional analytic study of both quantitative and qualitative methods. The objectives of the study were 1) to describe the level of Pre-Diabetes Mellitus among Myanmar migrant workers in Suratthani Province 2) to describe the level of Pre-Hypertension among Myanmar migrant workers in Suratthani Province 3) to evaluate the factors associated with Pre-Diabetes Mellitus among Myanmar migrant workers in Suratthani Province 4) to evaluate the factors associated with Pre-Hypertension among Myanmar migrant workers in Suratthani Province and 5) how to solve the problem through policy development support of those having Pre-Diabetes and Pre-Hypertension situation. Five hundred and eight samples of Myanmar migrant workers in Suratthani Province. In the first phase study (Quantitative study) data was collected data by interviews questionnaires; questionnaire structures were constructed based on literature reviews and were used for data collection. Cronbach’s alpha coefficient of stress, attitude, and practice were 0.78, 0.87, and 0.82, and the Kuder-Richardson coefficient of knowledge was 0.77. Multi-stage random sampling was used for sampling selection and data analysis was perfumed using Multiple logistic regression. The second phase (Qualitative study) collected data by focus group discussions and multi-disciplinary meetings. The results revealed that those workers who had a normal blood glucose level of 81.30% (95% CI: 77.66-84.46), had a blood glucose value at risk of 12.60% (95% CI: 9.97-15.78), and had a blood glucose level in the group requires of the clinical confirmation of a diagnosis by a physician of 6.10% (95% CI: 4.32-8.55). Those workers had blood pressure values in the normal group of 31.70% (95% CI: 27.78-35.88) and had blood pressure values at the risk group of 43.70% (95% CI: 39.43-48.06), had blood pressure values had to submit a doctor's diagnosis of 22.24% (95% CI: 18.83-26.07) and had to see a doctor within the same day 2.36% (95% CI: 1.34-15.78). Factors associated with Pre-Diabetes Mellitus consisted of graduating below junior high school students (Adj.OR=2.12; 95% CI: 1.09-4.10; p-value=0.026) married (Adj.OR=1.96; 95% CI: 1.04-3.70; p-value=0.037), unsupported media or materials in Myanmar language (Adj.OR=1.78; 95% CI: 1.11-2.85; p-value=0.016), and the problem of housing environment was moderate/high (Adj.OR=1.71; 95% CI: 1.05-2.77; p-value= 0.028). Factors associated with Pre-Hypertension including age >30 years (Adj.OR=2.77; 95% CI: 1.77-4.33; p-value<0.001), BMI on the overweight /obese scale. (Adj.OR=2.30; 95% CI: 1.50-3.53; p-value<0.001), average family payment per month ≥5,000 baht (Adj.OR=2.28; 95% CI: 1.17-4.44; p-value=0.015), not received health information (Adj.OR=2.28; 95% CI: 1.44-3.61; p-value<0.001), male (Adj.OR=1.97; 95% CI: 1.27-3.07; p-value=0.002), had duration stay in Thailand ≥48 months (Adj.OR=1.69; 95% CI: 1.06-2.71; p-value=0.027), and had work our per day ≤8 hours (Adj.OR=1.60; 95% CI: 1.03-2.49; p-value=0.036), respectively. In conclusion, the results of this study were found that almost a fifth of Myanmar migrant workers are at risk of Pre-Diabetes and two-thirds are at high blood pressure risk. Factors associated with Pre-diabetes Mellitus and Pre-Hypertension include personal and socioeconomic characteristics, Environment factors, and the health service system. These factors are thought to have a profound effect on Pre-diabetes Mellitus and Pre-Hypertension. The recommendations policies; 1) The development system of spatial management in the public health system should be developed for the special area to operate area-based public health for Myanmar migrant workers. 2) Integrating all health network partners in Suratthani Province and establishing a Provincial Health Promotion Committee for migrant workers is a very useful process for integrating resources for the development of the labor health care system.th_TH
