dc.description.abstractปัจจุบันนี้ โลกมีการเคลื่อนย้ายของประชากร (migration) เพิ่มขึ้นอย่างรวดเร็ว ซึ่งอาจจะเป็นการเคลื่อนย้ายด้วยเหตุผลที่หลากหลาย เช่น การลี้ภัยทางการเมือง การย้ายถิ่นที่อยู่เพื่อสร้างโอกาสในการทำธุรกิจ การทำงาน การศึกษา หรือการย้ายติดตามครอบครัว เป็นต้น ในปี พ.ศ. 2560 พบว่า ประมาณ 1 ใน 4 (ร้อยละ 25.7) ของจำนวนประชากรที่เคลื่อนย้ายระหว่างประเทศที่อาศัยอยู่ในต่างแดนเป็นระยะเวลาไม่น้อยกว่าสามเดือน ด้วยจุดประสงค์การทำงานหรือการตั้งครอบครัว หรือที่เรียกว่า Expatriates (Expats) คิดเป็นร้อยละ 0.9 ของประชากรโลก (7.60 พันล้าน) สำหรับในประเทศไทย ข้อมูลแรงงานต่างด้าวของสำนักบริหารแรงงานต่างด้าว กรมการจัดหางาน กระทรวงแรงงาน ระบุว่า ณ เดือนตุลาคม 2562 มีคนต่างด้าวที่ได้รับใบอนุญาตทำงานทั่วประเทศทั้งสิ้น 3,028,000 คน ส่วนใหญ่เป็นแรงงานต่างด้าวจากประเทศกัมพูชา ลาว เมียนมาและเวียดนาม (CLMV) ซึ่งมีอยู่ถึง 2,748,189 คน แต่เมื่อพิจารณาเฉพาะผู้ที่อยู่ในกลุ่มแรงงานที่มีฝีมือ (skilled workers) มีจำนวนทั้งสิ้น 166,896 คน (ร้อยละ 6.07) โดยมากกว่าครึ่งอาศัยอยู่ในกรุงเทพมหานคร 92,860 คน (ร้อยละ 55.64) และที่เหลือกระจายอยู่ตามภูมิภาคต่างๆ ประมาณ 74,030 คน (ร้อยละ 44.36) ซึ่งในจำนวนนี้ ผู้ที่อาศัยอยู่ในเขตพื้นที่ภาคตะวันออกเฉียงเหนือจะมีอยู่ประมาณ 5,330 คน (ร้อยละ 7.19) แต่อย่างไรก็ตาม จำนวนนี้เป็นการบันทึกเฉพาะผู้ที่ลงทะเบียนหรือได้รับใบอนุญาตประกอบอาชีพเท่านั้น อาจมีจำนวนไม่น้อยที่เดินทางเข้า-ออกเหมือนนักท่องเที่ยว อาศัยอยู่เป็นระยะเวลาสั้นๆ หรือมีคู่ครองเป็นคนไทย ซึ่งจะแต่งงานหรือไม่แต่งงานกันแต่อยู่เป็นคู่ครอง (partner) ซึ่งคนไทยมักจะเรียกคนเหล่านี้ว่า “เขยฝรั่ง” หรือ “เขยต่างชาติ” ถึงแม้แนวโน้มของการเคลื่อนย้ายข้ามพรมแดนของประชากรโลกมากขึ้นเรื่อยๆ และมีจำนวนชาวต่างชาติ หรือ Expats ในประเทศไทยจำนวนไม่น้อย แต่การศึกษาทำความเข้าใจกับพฤติกรรมสุขภาพและประเด็นปัญหาต่างๆ ที่เกี่ยวข้องกับสุขภาพของคนกลุ่มนี้ยังมีไม่มากนัก แม้จะมีบ้างก็มักจำกัดอยู่ในกลุ่มแรงงานต่างด้าวจากประเทศเพื่อนบ้าน ยิ่งเมื่อเป็น Expats ที่มาจากประเทศในกลุ่มทวีปยุโรปหรืออเมริกาที่มาตั้งถิ่นฐานและมีครอบครัวในประเทศไทย ยิ่งมีการศึกษาน้อยมาก การศึกษานี้จึงจัดทำขึ้นเพื่อสำรวจสถานการณ์ด้านสุขภาพ โดยเฉพาะในแง่ของการเข้าถึงบริการสุขภาพ การประกันสุขภาพ และความจำเป็นด้านสุขภาพที่ไม่ได้รับการตอบสนองของคนกลุ่มนี้ ซึ่งเป็นเรื่องที่มีความสำคัญยิ่งและน่าจะเกิดประโยชน์ต่อการดูแลด้านสาธารณสุขของประเทศไทย วิธีการศึกษา การศึกษานี้เป็นการศึกษาแบบผสมวิธี (mixed methods) เฉพาะกลุ่มชาวต่างชาติผู้เข้ามามีครอบครัวและพำนักอาศัยอยู่ในเขตภาคตะวันออกเฉียงเหนือของประเทศไทยเท่านั้น (ต่อไปเรียกว่า “เขยต่างชาติ”) โดยเลือกพื้นที่ศึกษาแบบเจาะจงใน 3 จังหวัด ได้แก่ จังหวัดหนองบัวลำภู อุดรธานี และขอนแก่น ทำการเก็บข้อมูลทั้งในเชิงปริมาณ ได้แก่ การวิเคราะห์ข้อมูล 43 แฟ้มของกระทรวงสาธารณสุข ซึ่งเป็นข้อมูลผู้ป่วยที่มารับบริการผู้ป่วยนอกและผู้ป่วยในเป็นรายปีที่โรงพยาบาลภาครัฐทุกแห่งส่งไปยังคลังข้อมูลสุขภาพ (Health Data Center) ของกระทรวงฯ และการสำรวจโดยใช้แบบสอบถามที่ดัดแปลงมาจากการสำรวจอนามัยและสวัสดิการของสำนักงานสถิติแห่งชาติ (68 ข้อ) โดยผู้ตอบแบบสอบถามคือเขยต่างชาติจำนวนทั้งสิ้น 83 คน และเชิงคุณภาพ ได้แก่ การทบทวนวรรณกรรมที่เกี่ยวข้องทั้งในและต่างประเทศ การสัมภาษณ์เชิงลึกแบบรายบุคคลและรายกลุ่ม โดยการสัมภาษณ์แบบเจาะจงในผู้เกี่ยวข้องจำนวนทั้งสิ้น 42 คน ประกอบด้วย ผู้กำหนดนโยบายสาธารณสุข 6 คน, ผู้ทำงานเกี่ยวข้องกับชาวต่างชาติจากหน่วยงานอื่นๆ 4 คน (กระทรวงยุติธรรม, กระทรวงการต่างประเทศ, กระทรวงการพัฒนาสังคมและความมั่นคงของมนุษย์ และสำนักงานตรวจคนเข้าเมืองของสำนักงานตำรวจแห่งชาติ), นักพัฒนาองค์กรเอกชนที่ไม่แสวงหาผลกำไร (NGO) 1 คน, นักวิชาการ 3 คน, ผู้บริหารและปฏิบัติงานสาธารณสุขผู้เกี่ยวข้องกับงานบริการปฐมภูมิในพื้นที่ ได้แก่ รองนายแพทย์สาธารณสุขจังหวัด 1 คน, หัวหน้าสำนักงานสาธารณสุขอำเภอ 1 คน, นักวิชาการสาธารณสุข 11 คน, พยาบาลวิชาชีพ 4 คน, เจ้าหน้าที่ผู้ปฏิบัติงานวิเทศสัมพันธ์ของโรงพยาบาลเอกชน 2 คน และชายชาวต่างชาติที่เข้ามามีครอบครัวและพำนักอาศัยอยู่ในพื้นที่ศึกษา 9 คน ซึ่งมีสัญชาติแตกต่างกัน ได้แก่ ฝรั่งเศส 2 คน, อังกฤษ 2 คน, สวีเดน 1 คน, อเมริกัน 1 คน, ออสเตรเลีย 1 คน, สวิสเซอร์แลนด์ 1 คน และแคนาดา 1 คนth_TH

dc.description.abstractalternativeCurrently, there is a rapid increase in the international migration around the world. This may be due to various reasons, such as political situation in their home countries (refugees or asylum seekers), job or business opportunity, works, studies, or to start and/or to live with families. In 2017, it was found that approximately one fourth (25.7%) of international migrants resided in overseas more than 3 months with the purpose of working or starting families, so-called “Expatriates (Expats)”, accounted for 0.9 of the world population (7.6 billion). In Thailand, according to the data of Foreign Workers Administration Office, Department of Employment, Ministry of Labour (as of October 2019), there were foreigners who obtained a working permit in the country totally 3,028,000 people. Most of them were migrant workers from Cambodia, Lao PDR, Myanmar, and Vietnam (CLMV), which were 2,748,189 people. When considering only skilled workers (excluding CLMV workers), there were 166,896 people (6.07%). More than half (92,860 people; 55.64%) of these skilled workers lived in Bangkok, while the rest were scattering in other regions (74,030 people; 44.36%). Of those who lived in other regions, 7.19% (5,330 people) lived in the north-eastern region. However, this figure was the record of foreigners with a work permit only. There would have been many foreigners that come in and out as a tourist and temporarily stay in the country by marrying Thai women or living with Thai partners, whom Thai people called “Keui Farang” -- which literally means “Caucasian son-in-law” – with the term “Farang” referring to Caucasians. Although there are increasing trends of international migration globally as well as substantial numbers of Expats in Thailand, there are limited studies to investigate health and well-being or health related issues among these people. There may be some studies about neighboring migrant workers; however, the studies about Expats, particularly those come from Europe or America who have families in Thailand are very scarce. This study aimed to explore the situation of Expats’ health, especially health service utilization, health insurance, and unmet need. This information is very important and useful for health systems design in responsive to health need of this group of people in Thailand. Methods This study employed a mixed method design, focusing on Expats who came to have families and live in the northeast of Thailand only or son-in-law foreigners (not only westerners as there might have been male Expats from other places e.g., Middle-eastern, Asian, or Asia-pacific countries). The purposively selected study areas comprised three provinces: Nong Bua Lam Phu, Udon Thani, and Khon Kaen. Quantitative data were an analysis of 43 files (Health service data of public hospitals that are annually submitted to the Health Data Center of the Ministry of Public Health (MOPH)). Also, the health survey among son-in-law foreigners by using the survey questionnaire (68 questions) adapted from the Health Welfare Survey questionnaires of the National Statistics, which consisted of 83 respondents. Qualitative data included the reviews of literature conducted in Thailand and elsewhere, and the in-depth interviews and focus group interviews purposively conducted among 42 participants. interviewees comprising 6 MOPH policy makers, 4 representatives from other ministries that deal with foreigner-related work (Ministry of Justice, Ministry of Foreign Affairs, Ministry of Social Development and Human Security, and Immigration Bureau of the Royal Thai Police Headquarters), 1 NGO staff, 3 academics, 1 Deputy Director of a Provincial Health Office, 1 Director of a District Health Office, 11 public health officers, 4 registered nurses, 2 staff from the international affairs unit of a private hospital, and 9 expatriates comprising 6 different nationalities (2 French, 2 English, 1 Swedish, 1 American, 1 Australian, 1 Swiss and 1 Canadian).th_TH
