dc.description.abstractการวิจัยครั้งนี้ใช้รูปแบบการวิจัยและพัฒนา (Research and Development) มีวัตถุประสงค์เพื่อพัฒนาข้อเสนอแนะเชิงนโยบายรูปแบบการจัดการสุขภาพชุมชนหลังภาวะวิกฤตการระบาดของโรคโควิด-19 (Post Crisis Management) ของอาสาสมัครสาธารณสุขประจำหมู่บ้าน (อสม.) ตามแนวทางการจัดระบบบริการทางการแพทย์ปกติวิถีใหม่ (New Normal Medical Services) เก็บข้อมูลทั้งเชิงปริมาณและเชิงคุณภาพจากกลุ่มตัวอย่าง คือ อสม. และกลุ่มผู้มีส่วนได้เสียที่เกี่ยวข้องกับการปฏิบัติงานจัดการสุขภาพชุมชนหลังวิกฤตการระบาดของโรคโควิด-19 ในเขตสุขภาพที่ 5 และ 6 โดยแบ่งกลุ่มตัวอย่างตามกรอบแนวคิดการวิจัยนี้โดยอ้างอิงจากกรอบแนวคิดเชิงนิเวศวิทยาสังคม (Social Ecological Model) ซึ่งประกอบด้วยปัจจัยต่างๆ ทั้ง 5 ระดับ ได้แก่ 1) ปัจจัยส่วนบุคคล (Intrapersonal Factors) คือปัจจัยต่างๆ ในตัวของ อสม. ต่อพฤติกรรมหรือทักษะในการปฏิบัติงาน ได้แก่ ปัจจัยด้านความรู้ (Knowledge) เจตคติ (Attitude) และทักษะ (Skills) หรือ KAS ของ อสม. 2) ปัจจัยระหว่างบุคคล (Interpersonal Factors) คือปัจจัยที่เกิดจากกลุ่มคน หรือสิ่งแวดล้อมที่อยู่รอบๆ ตัวบุคคลคนนั้น ซึ่งส่งผลต่อการทำงานของ อสม. ได้แก่ กลุ่มเพื่อนของ อสม. ผู้รับบริการในชุมชนจากการทำงานของ อสม. และครอบครัวของ อสม. ซึ่งแสดงภาพการทำงานของ อสม. ด้านการให้ชุมชนมีส่วนร่วม (Community Engagement) 3) ปัจจัยด้านชุมชน (Community Factors) คือปัจจัยด้านทรัพยากรหรือทุนในชุมชน (Social Capitals) ซึ่งได้แก่ ทุนทางสังคม ทุนมนุษย์ ทุนทางกายภาพ ทุนในรูปเงินและทุนทางธรรมชาติ ที่ช่วยให้ อสม. ปฏิบัติงานได้อย่างมีประสิทธิภาพ ร่วมกับกลุ่มผู้มีส่วนได้เสียของชุมชน ได้แก่ คณะกรรมการพัฒนาคุณภาพชีวิตระดับอำเภอ (พชอ.) หรือ คณะกรรมการพัฒนาคุณภาพชีวิตระดับตำบล (พชต.) ของแต่ละพื้นที่ 4) ปัจจัยด้านองค์กร (Organizational Factors) หมายถึงปัจจัยในด้านหน่วยงานต่างๆ ที่อยู่ในพื้นที่ซึ่ง อสม. ปฏิบัติงานหรือสังกัดและมีความเกี่ยวข้องกับการบังคับบัญชาหรือควบคุมการปฏิบัติงานของ อสม. ให้เป็นไปตามมาตรฐาน เช่น เกณฑ์ประเมินต่างๆ ในพื้นที่ (KPIs) โดยกลุ่มผู้มีส่วนได้เสียในส่วนของปัจจัยด้านองค์กร ได้แก่ โรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) สำนักงานสาธารณสุขอำเภอ (สสอ.) สำนักงานสาธารณสุขจังหวัด (สสจ.) คณะกรรมการโรคติดต่อจังหวัด และผู้ตรวจราชการเขตสุขภาพทั้ง 2 เขต และ 5) สถานการณ์วิกฤตโลกและปัจจัยด้านนโยบายสาธารณะ (Global Crisis and Public Policy Factors) หมายถึงปัจจัยที่เกี่ยวข้องกับสถานการณ์ด้านต่างๆ ทั้งในและต่างประเทศ ซึ่งส่งผลกระทบถึงประเทศอื่นๆ ทั่วโลก รวมทั้งนโยบายสาธารณะที่รัฐบาลประกาศใช้ เช่น New Normal Medical Services โดยกลุ่มผู้มีส่วนได้เสียในการประเมินภาพรวมเหล่านี้ต่อการปฏิบัติงานของ อสม. คือ ผู้บริหารระดับสูงของกระทรวงสาธารณสุข จากกรมสนับสนุนบริการสุขภาพ กรมควบคุมโรคและกรมการแพทย์ การศึกษานี้เก็บข้อมูลระหว่างเดือนกรกฎาคม พ.ศ. 2564 ถึงเดือนกุมภาพันธ์ พ.ศ. 2565 การเก็บข้อมูลในส่วนของปัจจัย Intrapersonal และ Interpersonal Factors ใช้การเก็บข้อมูลทั้งเชิงปริมาณและคุณภาพ ส่วนปัจจัยด้านชุมชน องค์กร และสถานการณ์วิกฤตโลกและปัจจัยด้านนโยบายสาธารณะ เป็นการเก็บข้อมูลเชิงคุณภาพด้วยวิธีการสัมภาษณ์แบบกึ่งโครงสร้าง จากผลการศึกษาพบว่า กรอบการทำงานของ อสม. ก่อนภาวะวิกฤตการระบาดของโรคโควิด-19 นั้น ประกอบด้วย ทักษะการจัดการสุขภาพชุมชนของ อสม. ในด้านต่างๆ 4 ด้าน ได้แก่ 1) การเขียนและจัดทำโครงการ 2) การส่งเสริมสุขภาพและป้องกันโรค 3) การใช้เทคโนโลยีสารสนเทศด้านสุขภาพ และ 4) การเป็นผู้นำและสร้างการมีส่วนร่วมของชุมชน โดยหลังจากการพัฒนารูปแบบเพื่อสร้างคู่มือ อสม. นักจัดการสุขภาพชุมชนในภาวะวิกฤตหลังการระบาดของโรคโควิด-19 (Post Crisis Management) พบว่า บทบาทของ อสม. ด้านที่ 5 ได้แก่ การจัดการภาวะวิกฤตภายหลังสถานการณ์การระบาดของโรคโควิด-19 เป็นทักษะที่จำเป็นสำหรับ อสม. ในอนาคต เพื่อรองรับกับบริบทของปัญหาสาธารณสุขในอนาคตที่มีความซับซ้อนและไม่มีพรมแดนth_TH

dc.description.abstractalternativeThis study applied the research and development model aiming to propose policy recommendations for community health management approaches during the post COVID-19 crisis management for village health volunteers (VHVs) under the New Normal Medical Services policy. Both quantitative and qualitative data were obtained from the sample groups during the post COVID-19 pandemic, which consisted of the VHVs and the groups of stakeholders participating in community health management practices in both health regional zones 5 and 6. Samples were categorized using the Social Ecological Model-based study methodology as a framework. This comprises five levels; 1) Intrapersonal Factors are the collection of elements within the VHVs that influence behavior or performance abilities. These include knowledge, attitude, and skills or KAS of VHVs; 2) Interpersonal Factors or the environment surrounding VHVs that impacts their work performances, such as peer groups, patients and family members of the VHVs, reflecting their satisfaction towards VHVs' work performance with community participation; 3) Community Factors refer to community capitals including social, human, physical, financial, and natural capitals assisting VHVs in carrying out their duties efficiently in collaboration with community stakeholder groups, such as community committees, the District and the Subdistrict Quality of Life Development Committees in each neighborhood; 4) Organizational Factors refers to factors in sectors where the VHVs works or is associated and is related to the command or control lines. These include various key performance indexes (KPIs) with many groups of stakeholders including subdistrict health promoting hospitals, district and provincial health offices, the Provincial Communicable Disease Committees and inspectors of both health regional zones; and 5) Global Crisis and Policy refers to situations at home and abroad that impact other nations throughout the globe, including the public policies declared by governments. Stakeholders are top executives of the Ministry of Public Health from the Department of Health Service Support, the Department of Disease Control, and the Department of Medical Services. Data were collected between July 2021 and February 2022. Intrapersonal and Interpersonal Factors data gathering utilized both quantitative and qualitative data collecting methods. With regards to community, organizations, global crisis conditions and public policy factors, this study used semi-structured interviews to obtain qualitative data. According to the findings, the structure of the VHV prior to the COVID-19 pandemic consisted of the following four categories: 1) project writing and implementation, 2) health promotion and disease prevention, 3) use of health information technology, and 4) community leadership and participation. After designing a model to develop a handbook for VHVs, community health managers in post-crisis management, the responsibility of the VHVs in the fifth component was determined to be crisis management after the COVID-19 pandemic. It will be required for VHVs in the future to be able to deal with the context of future public health issues that are complicated and transnational.th_TH
