dc.description.abstractการศึกษาภาวะพหุสัณฐานของนิวคลีโอไทด์เดี่ยวที่พบบ่อยในผู้ป่วยโรคเรื้อรังของประเทศไทยโดยใช้ชิป เป็นโครงการที่ดำเนินการศึกษาในกลุ่มผู้ป่วยโรคเรื้อรัง จำนวน 1,800 ตัวอย่าง โดยในปีที่ 1 รับตัวอย่างจาก 4 โครงการ เพื่อดำเนินการสร้างข้อมูลพันธุกรรมมนุษย์ที่มีความละเอียดปานกลาง ได้แก่ ข้อมูลจีโนทัยป์ เพื่อให้เกิดชุดข้อมูลพันธุกรรมของชาวไทย ดังนี้ 1) โครงการวิจัย การศึกษาปัจจัยพันธุกรรมของกลุ่มประชากรระยะยาวสำหรับโครงการจีโนมิกส์ประเทศไทย จำนวน 849 ตัวอย่าง โดย ศ.นพ.รุ่งโรจน์ กฤตยพงษ์ และคณะผู้วิจัย 2) โครงการวิจัยความสัมพันธ์ระหว่างพหุสัญฐานและการกลายพันธุ์ที่สัมพันธ์กับโรคเบาหวานชนิดที่ 2 จำนวน 640 ตัวอย่าง โดย รศ.พญ.สมรักษ์ จึงสมาน และคณะผู้วิจัย 3) โครงการวิจัยการศึกษาขั้นสูงของลักษณะที่แสดงออกทางคลินิกและลักษณะทางพันธุกรรมในผู้ป่วยชาวไทยที่เป็นโรคคอเลสเตอรอลสูงทางพันธุกรรมเพื่อการประยุกต์ใช้ทางคลินิก จำนวน 73 ตัวอย่าง โดย ศ.นพ.วีรพันธุ์ โขวิฑูรกิจ และคณะผู้วิจัย 4) โครงการวิจัยการศึกษาความแปรผันทางพันธุกรรมในอาสาสมัครผู้ป่วยโรคอ้วนชาวไทย เพื่อพัฒนาคะแนนความเสี่ยงทางพันธุกรรมสำหรับประเมินความรุนแรงทางคลินิก และการตอบสนองต่อการรักษาอย่างจำเพาะบุคคล จำนวน 238 ตัวอย่าง โดย ดร.ชาติชาย ไชยชนะ และคณะผู้วิจัย โดยทำการถอดรหัสพันธุกรรมทั่วทั้งจีโนมในประชากรไทยด้วยเทคนิคไมโครอะเรย์โดยใช้ชิป ซึ่งเป็นแพลตฟอร์มของไมโครอะเรย์ที่มีความจำเพาะต่อประชากรที่มีเชื้อสายเอเชียตะวันออก เรียกว่า Asian Screening Array (ASA) ที่มีฐานข้อมูลพันธุกรรมชาวเอเชียที่มีจำนวนชาติพันธุ์ครอบคลุมภูมิภาคเอเชียตะวันออกเฉียงใต้ ด้วยแพลตฟอร์มใหม่นี้มีความใกล้เคียงกับชาติพันธุ์ของตัวอย่างประชากรชาวไทยที่จะนำมาจีโนทัยป์ ยิ่งชาติพันธุ์ใกล้เคียงกันมากผลการทำจีโนทัยป์จะยิ่งมีความแม่นยำจากการตรวจสอบคุณภาพข้อมูล SNP genotype (SNP QC) จำนวน 659,184 ตำแหน่ง ด้วยโปรแกรมวิเคราะห์ GenomeStudio Software พบว่า มีจำนวน 1,760 ตัวอย่าง มีค่า Call rate > ร้อยละ 98 คือเป็นตัวอย่างที่ไม่มีข้อมูลการถอดรหัสพันธุกรรมขาดหายไป มากกว่า ร้อยละ 2 (2% missing data) และ จำนวน 40 ตัวอย่าง มีค่า Call rate < ร้อยละ 98 ตัวอย่างที่มีข้อมูลการถอดรหัสพันธุกรรมขาดหายไป มากกว่าร้อยละ 2 ตัวอย่างนั้นจะไม่ถูกนำมาวิเคราะห์ และจากการวิเคราะห์ข้อมูลลักษณะทางพันธุกรรม ด้วยโปรแกรมวิเคราะห์ PLINK โดยวิเคราะห์ความสมดุลประชากร ตาม Hardy Weinberg Equilibrium (HWE) ของข้อมูล SNP จำนวน 657,060 จากผู้ป่วยในโครงการวิจัย NCD ทั้ง 4 โครงการ พบว่า ส่วนใหญ่มีค่า P-value เท่ากับ 1 หมายถึง ไม่มีความแตกต่างกันในกลุ่มประชากร แปลว่าอยู่ในความสมดุล ในการศึกษานี้ จะยกตัวอย่างโรคที่พบบ่อยในคนไทยและความถี่อัลลีลของตัวอย่างกลุ่มยีนที่เกี่ยวข้องกับแต่ละโรค เพื่อเป็นประโยชน์ในการนำไปศึกษาเปรียบเทียบกับกลุ่มควบคุมต่อไป โรคภาวะไขมันในเลือดสูง Hypercholesterolemia มียีนที่พบรายงานจำนวนมาก ว่าเกี่ยวข้องกับการแสดงออกของโรค ได้แก่ LDLR, APOE และ PCSK9 พบความถี่อัลลีล (Minor Allele Frequency; MAF) ของทั้ง 3 ยีน และในโรคอ้วน (Obesity) มีการศึกษาเกี่ยวกับพันธุกรรมที่เกี่ยวข้องกับการแสดงออกของโรค โดยรวบรวมยีนจากการศึกษาด้วยวิธี Genome Wide Association Study (GWAS) ในประชากรเอเชียตะวันออก (Sun, Chang et al.) ในรายงานนี้ได้คัดเลือกกลุ่มยีนที่มีตำแหน่งพันธุกรรมที่ความแตกต่างอย่างมีนัยสำคัญ (P-value < 5.0E−8) บางตำแหน่งเพื่อรายงานความถี่อัลลีล ได้แก่ ยีน GIPR, FAM60A, CAMK1D, HERC4, RRP12, HLA-DRA, HLA-DRB5 และ QPCTL โรคความดันโลหิตสูง (Hypertension) มีการศึกษาเกี่ยวกับพันธุกรรมที่เกี่ยวข้องกับการแสดงออกของโรคด้วยวิธี Genome Wide Association Study (GWAS) ครั้งแรกในประชากรชาวยุโรปจำนวน 200,000 คน (International Consortium for Blood Pressure Genome-Wide Association Studies et al.) ในรายงานนี้ได้คัดเลือกกลุ่มยีนที่มีตำแหน่งพันธุกรรมที่ความแตกต่างอย่างมีนัยสำคัญ (P-value < 5.0E−8) บางตำแหน่งเพื่อรายงานความถี่อัลลีล ได้แก่ MOV10, SLC4A7, MECOM, SLC39A8, GUCY1A3 และ GUCY1B3 จากผลการวิเคราะห์เบื้องต้นของแต่ละกลุ่มโรค NCD ทำให้ทราบถึงลักษณะทางพันธุกรรมที่มีความสัมพันธ์ต่อการแสดงออกของโรค ซึ่งปัจจัยทางพันธุกรรมของผู้ป่วยกลุ่มโรค NCD จะต้องวิเคราะห์ร่วมกับลักษณะฟีโนไทป์ (Phenotype) และพฤติกรรมการดำเนินชีวิต การกิน การนอน การพักผ่อน การออกกำลังกายและการทำงานต่างๆ ของผู้ป่วย โดยข้อมูลเหล่านี้จะถูกวิเคราะห์แยกตามโครงการวิจัย เพื่อนำองค์ความรู้เหล่านี้ไปศึกษาต่อยอดในการวินิจฉัย ป้องกัน ควบคุมและรักษาโรคให้มีประสิทธิภาพในกลุ่มโรคไม่ติดต่อเรื้อรังในประเทศไทยต่อไปth_TH

dc.description.abstractalternativeGenomics Thailand Project: Database for Common Single Nucleotide Polymorphisms (SNPs) in Non Communicable Diseases (NCD) is a project to study NCD patients. In the first year, 1,800 samples were received from 4 projects as follows. 1) Genetic Study in patients with high risk for cardiovascular events compared to controls, 849 samples, by Prof.Dr. Rungroj Krittayaphong and researcher team (NCD_01). 2) Association of Type 2 Diabetes Mellitus related SNP genotypes, 640 samples, by Assoc.Prof. Somlak Chuengsamarn and researcher team (NCD_02). 3) Advanced clinical phenotyping and genotyping of Thai patients with familial hypercholesterolemia (FH) for clinical application, 73 samples, by Prof.Dr. Weerapan Khovidhunkit and researcher team (NCD_05). 4) Genetic variation study of obesity in Thai patients: To develop a polygenic risk score for clinical risk assessment and personalized medicinal treatment, 238 samples, by Dr. Chartchai Chaichana (NCD_06). SNP genotyping was performed using a chip-based microarray technique on an East Asian-specific platform known as the Asian Screening Array (ASA). This is based on information in the Asian genetic database, which includes data from ethnic groups in Southeast Asia. Quality control of 659,184 SNP genotypes (SNP QC) by the GenomeStudio Software analysis program found that 1,760 samples had a call rate > 98%, (less than 2% missing data). Another 40 samples had a call rate < 98% (more than 2% missing data) and will not be analyzed further. Hardy Weinberg Equilibrium (HWE) testing of 657,060 SNP genotypes from 4 NCD research projects found that most of them had a P-value of 1, which means there were no differences in the population, and that the population is in equilibrium. This study includes examples of NCDs in the Thai population and allele frequencies of genes associated with each disease. Genes with different reported allele frequencies in hypercholesterolemia include GIPR, FAM60A, CAMK1D, HERC4, RRP12, HLA-DRA, HLA-DRB5, and QPCTL genes (P-value < 5.0E−8). The MOV10, SLC4A7, MECOM, SLC39A8, GUCY1A3 and GUCY1B3 genes are associated with hypertension (P-value < 5.0E−8). These preliminary analyses of each group of NCD patients have provided insights into genetic aspects of the diseases. Genetic factors should be analysed in combination with physiological, environmental, and behavioural characteristics of patients. These data will be analysed separately for each research project. Knowledge gained will facilitate further studies into the effective diagnosis, prevention, control, and treatment of non-communicable diseases in Thailand.th_TH
