| dc.description.abstract | การวิจัยครั้งนี้เป็นการวิจัยเชิงปฏิบัติการแบบมีส่วนร่วมและการพัฒนา เพื่อศึกาษารูปแบบและพัฒนาการจัดระบบบริการสุขภาพระดับปฐมภูมิของศูนย์สุขภาพชุมชนหนองตอง อำเภอหางดง จังหวัดเชียงใหม่ โดยศึกษาจากผู้ให้บริการ ผู้รับบริการ (ผู้นำชุมชน และประชาชน) รวบรวมข้อมูลโดยการสนทนากลุ่ม การสัมภาษณ์เจาะลึก การสังเกต สำหรับการพัฒนาระบบบริการสุขภาพ ใช้กระบวนการมีส่วนร่วม โดยการระดมความคิดเห็นและสะท้อนปัญหาและความต้องการ การดำเนินการพัฒนาศักยภาพและบุคลากร และพัฒนาระบบบริการสุขภาพ วิเคราะห์ข้อมูลเชิงเนื้อหา ผลการวิจัย รูปแบบบริการและการปฏิบัติงานของบุคลากรไม่เป็นไปตามมาตรฐาน การจัดระบบบริการสุขภาพระดับปฐมภูมิ เช่น ระบบการทำงานไม่คล่องตัว อุปกรณ์ไม่เพียงพอ สถานที่บางส่วนไม่เหมาะสม ความร่วมมือจากองค์กรท้องถิ่นและการสนับสนุนจากอาสาสมัครสาธารณสุข (อสม.) มีน้อย จากปัญหาดังกล่าวได้มีการพัฒนาระบบบริการสุขภาพของศูนย์สุขภาพชุมชนแบบมีส่วนร่วมโดยการพัฒนาบุคลากร การจัดการระบบบริการสุขภาพ และปรับปรุงสถานที่ให้มีความคล่องตัว จัดหาอุปกรณ์ในการให้บริการ การสร้างความเข้าใจและความเข้มแข็งให้กับชุมชนด้านสุขภาพ การปรับทัศนคติของประชาชนต่อระบบบริการ การสร้างความมั่นใจในการให้บริการตามบทบาทของ อสม. การสนับสนุนโครงการส่งเสริมสุขภาพของประชาชน ทำให้ศักยภาพของบุคลากรเพิ่มมากขึ้น มีการปรับปรุงสถานที่ให้เหมาะสม ได้รับความร่วมมือจากเทศบาลตำบล อสม. และผู้นำกลุ่มต่างๆ ในชุมชนเป็นอย่างดี ทั้งด้านงบประมาณและการร่วมกิจกรรมทางด้านสุขภาพ เช่น ชมรมผู้สูงอายุ ชมรมคนรักสุขภาพ ชมรมนวดแผนไทย การให้บริการสาธารณสุขเคลื่อนที่ การอบรมผู้เสพยาเสพติด การป้องกันไข้เลือดออก ฯลฯ ข้อเสนอแนะ การศึกษาครั้งนี้จะพบว่าการพัฒนาระบบบริการสุขภาพจะสำเร็จได้ต้องใช้กระบวนการมีส่วนร่วมและใช้วิธีการต่างๆ ผสมผสานกัน ได้แก่ การฝึกอบรม การนิเทศ การจัดเวทีร่วมคิด การให้คำแนะนำ และการเรียนรู้จากพื้นที่ที่ประสบความสำเร็จ | th_TH |

| dc.description.abstractalternative | This participatory action research aimed to study model for the to development of a primary health care service system at NhongTong Primary Care Unit, Hangdong District, Chiang Mai Province. Data were collected from health personnel and clients (community leaders and people), by focus group discussion, in-depth interview, and observation.The participatory approach was focused by brain-stroming, reflecting on problems and needs, capacity building of health personnel, and development of the health care service system. Content analysis was used for data analysis. Results: It was found that the health care service system did not reach the standard for a primary care service system. For example, the management system did not function well, equipment was inadequate, and office space was not well organized, health personnels lacked of confidence in providing proactive services and primary medical care and participation from local authorities and village health volunteers was limited. As the result of the findings, health personnel were trained, health service management was conducted and office space was organized and equipment provided. Furthermore, understanding was created and empowered the community, attitudes were changed towards the health service among people in the communities, learning from best practice communities, the village health volunteers were encouraged to be more confident, and was provided support for health promoting activities of the people. As a result, health personnel have developed their potential as follows: office space was better organized, cooperation from local authorities, village health volunteers, and community leaders has been increased in terms of budgeting and participating in community activities such as elderly clubs, health promoting clubs, Thai massage clubs, mobile clinics, training of drug-users, and the prevention of hemorrhagic fever. Suggestion: The results of the study suggested that the development of a health care service system will be successful by using a participatory approach and integrated methods such as training, supervision, conducting forums, suggestions and learning from best practice communities. | en_US |
