dc.description.abstractการวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาลักษณะของการอภิบาลระบบสุขภาพปฐมภูมิใน 2 ระดับ คือ (1) ระดับพื้นที่ ที่มีโรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) เป็นแกนกลางในการขับเคลื่อน และ (2) ระดับภาพรวมทั้งจังหวัดที่มีองค์การบริหารส่วนจังหวัด (อบจ.) เป็นแกนกลางในการขับเคลื่อนว่ามีลักษณะเป็นอย่างไร มีความสอดคล้องกับหลักการอภิบาลที่องค์การอนามัยโลกเสนอแนะ และได้รับการขานรับจากกระทรวงสาธารณสุขหรือไม่ มีการเปลี่ยนแปลงลักษณะสำคัญอะไรไปบ้าง มีปัญหาอุปสรรค หรือข้อเด่นในการอภิบาลอย่างไรบ้าง และมีข้อเสนอแนะแนวทางในการพัฒนาอย่างไร ในด้านวิธีวิทยาการวิจัยใช้การวิจัยแบบผสมผสานโดยใช้ตัวอย่างการวิจัยจาก 32 รพ.สต. 8 จังหวัด 4 ภาค เก็บรวบรวมข้อมูลโดยแบบสำรวจ การสัมภาษณ์ และการสัมมนา ผลการศึกษา พบว่า ก่อน และหลังการถ่ายโอนการอภิบาลระบบสุขภาพปฐมภูมิในระดับพื้นที่ของ รพ.สต. ยังมีสภาพเหมือนก่อนการถ่ายโอน ส่วนภาพรวมในระดับจังหวัด และระดับชาติ โครงสร้างการอภิบาลแตกต่างไปจากเดิม โดยกระทรวงสาธารณสุขยังมีฐานะที่เป็นแกนนำในความรับผิดชอบในการดูแลสุขภาวะของชาติ และมี อบจ. เป็นองค์กรนำด้านการอภิบาลสาธารณสุขในระดับจังหวัด ร่วมกับกระทรวงสาธารณสุขผ่านคณะกรรมการสุขภาพระดับพื้นที่ (กสพ.) ที่เป็นกลไกสำคัญในการขับเคลื่อนระบบสุขภาพปฐมภูมิโดยหลังจาก รพ.สต. ถ่ายโอนมายัง อบจ. ระบบการอภิบาลในระดับพื้นที่ และระดับจังหวัดยังมีปัญหาอุปสรรคในด้านการทำงานร่วมกันอันเนื่องจากทัศนคติ และกฎระเบียบที่ไม่เอื้อต่อสถานการณ์ของแต่ละหน่วยงาน สำหรับแนวทางการพัฒนาการอภิบาลระบบสุขภาพปฐมภูมิที่ควรเป็น จำเป็นต้องอาศัยเอกภาพภายใต้โครงสร้างแบบพันธมิตรร่วมมือ หรือการทำงานแบบภาคีเครือข่ายทั้งระดับพื้นที่จังหวัด และระดับชาติ โดยจัดโครงสร้างคณะกรรมการทำงานในสัดส่วนที่เหมาะสมth_TH

dc.description.abstractalternativeThis research aims to study the characteristics of primary health care systems at 2 levels: (1) the area level with Subdistrict Health Promotion Hospitals as the main driving force and (2) the overall level for the entire province with Provincial Administrative Organizations. It is the core that drives what it looks like Is it consistent with the principles of pastoral care recommended by the WHO and received by the ministry of public health? What important characteristics have changed? What are some problems, obstacles, or outstanding points in pastoral care? And what are the suggestions for development guidelines? In terms of research methodology, mixed methods research was used. using research samples from 32 samples, 8 provinces, 4 regions, data were collected by surveys, interviews, and seminars. The results of the study found that Before and after the transfer, the primary health care system at the local level of the subdistrict health promotion hospital is still in the same condition as before the transfer. As for the overall picture at the provincial and national level, the governance structure is different from before. That is Ministry of Public Health still has a leading position in responsibility for caring for the healthiness of the nation, and the PAOs is the leading organization for caring at the provincial level through the Committee for Health Area level (CHA), which is an important mechanism in driving the primary health system after the subdistrict health promotion hospital was transferred to the provincial administrative organization, The administrative systems at the local and provincial levels still had obstacles in working together due to attitudes and regulations that were not conducive to the situation of each agency. As for the development guidelines for primary health care systems, it should be based on unity under a cooperative alliance structure or working as a network partner at the local, provincial, and national levels by structuring working committees in appropriate proportions.th_TH
