| dc.description.abstract | บทนำ: การติดเชื้อในโรงพยาบาล (healthcare-associated infection, HAI) การติดเชื้อดื้อยาต้านจุลชีพ (antimicrobial resistance, AMR) และการสั่งใช้ยาต้านจุลชีพอย่างไม่เหมาะสม (inappropriate antimicrobial use, AMU) เป็นปัญหาสาธารณสุขสำคัญของประเทศไทย จึงจำเป็นต้องมีฐานข้อมูลเชิงประจักษ์ระดับชาติที่บูรณาการทั้งภาระโรค ภาระเชื้อดื้อยา รูปแบบการใช้ยา และความพร้อมของระบบป้องกันและควบคุมการติดเชื้อในโรงพยาบาล (infection prevention and control, IPC) และระบบสนับสนุนการใช้ยาต้านจุลชีพอย่างสมเหตุผล (antimicrobial stewardship, AMS) เพื่อพัฒนาระบบเฝ้าระวัง การกำหนดนโยบาย และการขับเคลื่อนแผนยุทธศาสตร์ของประเทศ วัตถุประสงค์: เพื่อหาความชุกและปัจจัยที่สัมพันธ์กับ HAI AMR และ AMU ผลกระทบทางคลินิกและภาระค่าใช้จ่าย รวมทั้งประเมินระบบ IPC และ AMS ในโรงพยาบาล 100 แห่งของประเทศไทย วิธีการศึกษา: การวิจัยเชิงผสมผสาน ดำเนินการในโรงพยาบาล 100 แห่ง ครอบคลุม 13 เขตสุขภาพ ระหว่างเดือนกันยายน พ.ศ. 2568 ถึง มกราคม พ.ศ. 2569 ประกอบด้วยการสำรวจความชุก ณ จุดเวลา การวิเคราะห์ปัจจัยที่สัมพันธ์ด้วยการวิเคราะห์ถดถอยโลจิสติกพหุตัวแปร และประเมินระบบ IPC และ AMS โดยใช้ WHO Infection Prevention and Control Assessment Framework (WHO IPCAF), WHO Hand Hygiene Self-Assessment Framework 2010 (WHO HHSAF 2010) และคู่มือการประเมินการจัดการการดื้อยาต้านจุลชีพอย่างบูรณาการในโรงพยาบาล (EE-AMR Tool, Thailand) ร่วมกับสัมภาษณ์และสังเกตการปฏิบัติงานขณะดูแลผู้ป่วย HAI ผลการศึกษา: ผู้ป่วยที่เข้าเกณฑ์การสำรวจ 29,414 ราย พบความชุก HAI ร้อยละ 2.9 (95% confidence interval [CI] 2.7–3.1) สูงสุดในโรงพยาบาลมหาวิทยาลัย (ร้อยละ 6.4) และในหอผู้ป่วยวิกฤต (ร้อยละ 9.0) ตำแหน่งติดเชื้อที่พบสูงสุด คือ ทางเดินหายใจ (ร้อยละ 43.6) ปัจจัยที่สัมพันธ์กับ HAI สูงสุด คือ ระยะเวลานอนโรงพยาบาลถึงวันสำรวจ ≥7 วัน (adjusted odds ratio [aOR] 10.22; 95% CI 8.28-12.62) ความชุก AMR ตามกรอบ WHO GLASS และ ESKAPEE พบร้อยละ 48.0 (95% CI 46.1–50.0) โดยกลุ่ม HAI สูงกว่ากลุ่ม Non-HAI อย่างมีนัยสำคัญ (ร้อยละ 62.6 เทียบกับร้อยละ 43.7) ปัจจัยที่สัมพันธ์กับ AMR สูงสุด คือ การรักษาตัวในหอผู้ป่วยทารกแรกเกิด (aOR 3.80; 95% CI 1.86-7.80) ส่วนความชุก AMU พบร้อยละ 48.1 (95% CI 47.6-48.7) โดยมีสัดส่วนการสั่งใช้ยากลุ่ม Watch ร้อยละ 62.2 และพบการใช้ยาต้านจุลชีพไม่เหมาะสมร้อยละ 29.6 โดยการให้ยาป้องกันทางศัลยกรรมเป็นปัจจัยที่สัมพันธ์กับการสั่งใช้ยาไม่เหมาะสมสูงสุด (aOR 12.18; 95% CI 10.77–13.77) โดยภาพรวมพบว่ากลุ่ม HAI มีอัตราการเสียชีวิตที่ 90 วันหลังสำรวจสูงกว่ากลุ่ม Non-HAI (ร้อยละ 23.3 เทียบกับร้อยละ 5.3) ระยะเวลานอนโรงพยาบาลยาวนานกว่าเกือบ 5 เท่า และค่าใช้จ่ายรวมสูงกว่า 7.7 เท่า นอกจากนี้ในผู้ป่วย AMR, AMU และผู้ป่วยที่ได้รับการสั่งใช้ยาต้านจุลชีพอย่างไม่เหมาะสมก็มีผลกระทบทางคลินิกเช่นเดียวกันผลการประเมินระบบ IPC ตามเกณฑ์ WHO IPCAF มีคะแนนเฉลี่ย 669.5 ± 84.8 จากคะแนนเต็ม 800 (ระดับ Advanced) แต่ยังมีช่องว่างด้านภาระงาน อัตรากำลังและอัตราการครองเตียง การฝึกอบรมและโครงสร้างพื้นฐานสนับสนุนด้าน IPC ผลการประเมิน WHO HHSAF 2010 มีคะแนนเฉลี่ย 378.0 ± 86.5 จาก 500 (ระดับ Advanced) โดยช่องว่างที่สำคัญ คือ บรรยากาศความปลอดภัยขององค์กร สำหรับผลการประเมินตามแบบประเมิน EE-AMR Tool มีคะแนนเฉลี่ยต่ำกว่าที่ 313.3 ± 85.6 จาก 500 (ระดับ Intermediate) ช่องว่างที่สำคัญ คือ การวิเคราะห์และสังเคราะห์ข้อมูลอย่างบูรณาการ และความเข้มแข็งของกลไกบริหารจัดการ AMR ซึ่งสอดคล้องกับผลการสัมภาษณ์เชิงลึก ผลการประเมินหน้างานโดยการสังเกตการณ์และการสัมภาษณ์พบว่าภาพรวมปฏิบัติได้ดี แต่ยังพบช่องว่างในเรื่องสุขอนามัยมือ การใช้อุปกรณ์ป้องกันส่วนบุคคล และมาตรการป้องกันการแพร่เชื้อทางอากาศ การติดตามการปฏิบัติตามมาตรการอย่างสม่ำเสมอ รวมถึงการเอาอุปกรณ์ทางการแพทย์ออกเมื่อหมดความจำเป็น สรุป: การศึกษานี้แสดงให้เห็นว่า HAI AMR และ AMU เป็นปัญหาที่เชื่อมโยงกันในระบบโรงพยาบาลไทย ผลการศึกษานี้สามารถใช้เป็นฐานข้อมูลระดับประเทศสำหรับการพัฒนาระบบเฝ้าระวัง การกำหนดมาตรการ IPC และ AMS รวมถึงจัดสรรทรัพยากรเพื่อยกระดับความปลอดภัยของผู้ป่วยและการจัดการเชื้อดื้อยาต้านจุลชีพในประเทศไทย | th_TH |

| dc.description.abstractalternative | Background: Healthcare-associated infections (HAIs), antimicrobial resistance (AMR), and
inappropriate antimicrobial use (AMU) pose major public health threats in Thailand, yet national
evidence that integrates disease burden, resistance, prescribing, and the maturity of infection
prevention and control (IPC) and antimicrobial stewardship (AMS) systems is lacking.
Objectives: To estimate the prevalence, risk factors, and impacts of HAI, AMR, and AMU and to
evaluate the implementation maturity of IPC and AMS systems in 100 hospitals across Thailand.
Methods: A mixed-methods study was conducted in 100 hospitals across Thailand between
September 2025 to January 2026. A point prevalence survey captured clinical data, with
multivariable logistic regression used to identify associated factors. IPC and AMS systems were
evaluated using the WHO IPCAF, WHO HHSAF 2010, and EE-AMR Tool, complemented by direct
observation and interviews.
Results: Among 29,414 surveyed inpatients, HAI prevalence was 2.9% (95% confidence interval
[CI] 2.7–3.1), highest in university hospitals (6.4%) and intensive care units (9.0%). Respiratory tract
infections were the most common HAI type (43.6%). A length of stay ≥7 days carried the strongest
association with HAI (adjusted odds ratio [aOR] 10.22; 95% CI 8.28–12.62). AMR prevalence based
on the WHO GLASS and ESKAPEE frameworks was 48.0% (95% CI 46.1–50.0) and was significantly
higher among HAI than non-HAI isolates (62.6% vs. 43.7%). Neonatal ward admission was the
strongest associated factor for AMR (aOR 3.80; 95% CI 1.86–7.80) ( aOR 3.80; 95% CI 1.86-7.80).
AMU prevalence was 48.1% (95% CI 47.6–48.7), with Watch-group antibiotics accounting for 62.2%
of prescriptions and 29.6% deemed inappropriate. Surgical antimicrobial prophylaxis was the
strongest independent factor (aOR 12.18; 95% CI 10.77–13.77). Compared with non-HAI patients,
those with HAIs had higher 90-day mortality (23.3% vs. 5.3%), nearly five-fold longer length of
stay, and 7.7-fold greater total costs.
Regarding IPC and AMS evaluation, Mean scores were 669.5 ± 84.8/800 for WHO IPCAF (Advanced),
378.0 ± 86.5/500 for WHO HHSAF 2010 (Advanced), and 313.3 ± 85.6/500 for the EE-AMR Tool
(Intermediate), with marked disparities between regional referral hospitals and community
hospitals. Key gaps included workload and staffing, institutional safety climate, integrated data
analysis, and AMR governance. On-site assessment revealed overall acceptable practice but
persistent gaps in hand hygiene, appropriate use of personal protective equipment, airborne
transmission precautions, timely removal of invasive devices once no longer required, and
consistent monitoring of compliance.
Conclusion: HAIs, AMR, and AMU are interconnected challenges in Thai hospitals. These findings
provide a national evidence base for strengthening surveillance, formulating IPC and AMS
measures, and prioritizing resource allocation to enhance patient safety and AMR containment in
