dc.description.abstractรายจ่ายด้านยาเป็นสัดส่วนสำคัญในระบบบัญชีรายจ่ายสุขภาพแห่งชาติของทุกประเทศ การติดตามรายจ่ายด้านยาอย่างต่อเนื่องเป็นเป้าหมายของการวิจัยนี้ การพัฒนาแพลตฟอร์มระบบการจัดทำบัญชีรายจ่ายด้านยาแห่งชาติ (ระยะที่ 1) ออกแบบสำหรับการดำเนิน การวิเคราะห์รายจ่ายด้านยาและการจัดทำบัญชีรายจ่ายด้านยาของประเทศไทยที่ใช้ข้อมูลเพื่อการบริหารยาระดับประเทศที่สามารถทำได้อย่างกึ่งอัตโนมัติ ต่อเนื่องและเป็นปัจจุบัน จึงมีความจำเป็นต้องใช้ระเบียบวิธีวิจัยทบทวนวรรณกรรมต่างประเทศและประเทศไทย สัมภาษณ์เชิงลึก/ประชุมกลุ่มกับผู้มีประสบการณ์และผู้มีส่วนเกี่ยวข้องกับข้อมูลระบบยาของประเทศไทย ทั้งภาครัฐและภาคเอกชน สืบค้นได้เอกสารวิชาการนานาชาติ และเอกสารวิชาการภายในประเทศ 22 การศึกษา ให้ทิศทางที่ชัดเจนในการทำแพลตฟอร์มระบบการจัดทำบัญชีรายจ่ายด้านยาระดับประเทศในด้านการกำหนดราคาเป็น 4 ระดับ คือ ราคาผู้ผลิต ราคาขายจากผู้ผลิตสู่ผู้กระจายยา ราคาที่องค์กรหลักประกันสุขภาพเป็นผู้จ่าย และราคายาที่ผู้บริโภคเป็นผู้จ่าย ทำการสัมภาษณ์เชิงลึก/ประชุมกลุ่มกับผู้เป็นแหล่งข้อมูลสำคัญของการได้มาของราคาและปริมาณยาทั้ง 4 ระดับ ได้แก่ สำนักงานคณะกรรมการอาหารและยา (อย.) (ทั้งยาทั่วไป และวัตถุเสพติด) องค์การเภสัชกรรม (อภ.) โรงพยาบาลราชวิถี องค์การบริหารส่วนจังหวัด (อบจ.) เทศบาล มูลนิธิสาธารณสุขแห่งชาติ (มสช.) บริษัทเอกชน และนักวิจัยระบบบัญชีรายจ่ายด้านยาและด้านรหัสยา ให้ผลการเชื่อมโยงของห่วงโซ่อุปทานด้านยาเป็นขั้นเริ่มต้นการผลิตยาหรือนำเข้ายา ขั้นผู้จัดจำหน่าย/ผู้กระจายยาขั้นผู้ค้าส่ง และขั้นผู้สั่งยา/ผู้ขายยา ผลการวิจัยระยะที่ 1 จะเป็นวิธีทำงานเพื่อให้มีข้อมูลประกอบการตัดสินใจเชิงนโยบายในการกระจายงบประมาณอย่างต่อเนื่องต่อไปth_TH

dc.description.abstractalternativeDrug expenditures take a significant proportion of all nations’ national health accounts. The continuous tracking national drug account system is the goal of this multiphase research. Phase 1 started with the carefully designed methodologies of how to build up the semi automated flows of legally bound administrative data at the national level for continuous and like real time use. From the systematic review of both international and Thai literature found 22 studies guiding the clarification of drug expenditures related to 4 levels of drug prices: producer, wholesaler, reimbursement, and consumer prices. Key informants related to 4 levels of drug prices were participating in in depth interviews group discussions, such as Food and Drug Administration for medicines in general and narcotics)), Government Pharmaceutical Organization, Rajavithi Hospital, provincial administrative organization, municipality, National Health Foundation, and researchers on national drug accounts and medicine coding. The content analysis categorized 4 tiers of supply chain contributing to success of national drug account study: tier 1 manufacturer importer, tier 2 distributer, tier 3 wholesaler, and tier N 1 prescriber over the counter seller while tier N = end user). These findings would feed to subsequent phases of the national drug account study to establish the data system for drug policy.th_TH
