dc.description.abstractการศึกษาครั้งนี้มีวัตถุประสงค์เพื่อพัฒนาหลักเกณฑ์และแนวทางการกำหนดความกันดารของพื้นที่ระดับอำเภอ เพื่อใช้อ้างอิงประกอบการจ่ายค่าตอบแทน “เบี้ยเลี้ยงเหมาจ่าย” แก่บุคลากรสาธารณสุข ทั้งนี้เป็นส่วนหนึ่งของรายงานข้อเสนอการปรับค่าตอบแทนบุคลากรสาธารณสุข เพื่อสนับสนุนการกระจายและเพิ่มประสิทธิภาพบุคลากรสาธารณสุขในการบริการประชาชน ที่สถาบันวิจัยระบบสาธารณสุขเสนอต่อกระทรวงสาธารณสุข หลักเกณฑ์การกำหนดพื้นที่กันดารดังกล่าวครอบคลุมประเด็นความยากลำบากในการเดินทาง สภาพความเป็นเมือง (city-life effect) และความเจริญในพื้นที่ ผลการศึกษาพบว่า การกำหนดความกันดารของพื้นที่ตามปัจจัยดังกล่าวตามเกณฑ์วัดที่กำหนด มีความสอดคล้องกับความเห็นของนายแพทย์สาธารณสุขจังหวัดร้อยละ 69.8 แนวทางการกำหนดความกันดารของพื้นที่ที่เริ่มต้นด้วยการใช้ข้อมูลคะแนนความกันดารของพื้นที่ ยืนยันด้วยความเห็นของนายแพทย์สาธารณสุขจังหวัดและผู้ตรวจราชการกระทรวงสาธารณสุข พบข้อจำกัดในการดำเนินงานที่น่าจะสามารถพัฒนาได้ในโอกาสต่อไปen_US

dc.description.abstractalternativeThis study was aimed at developing criteria and guidelines for identification and classification of hardship areas. This was part of a study on “Reform of Financial Incentives for Public Health Personnel” submitted to the Ministry of Public Health, Thailand. Criteria to identify hardship areas included transportation, city-life effect and economic development. It was found that, in classification of hardship areas using developed criteria, 69.8 percent corresponded with the opinions of the provincial chief medical officers. A guideline to classify hardship areas using hardship scores and confirmation by using opinions of the provincial chief medical officers and health inspectors involved some difficulties and limitations during implementation. There is a need to improve this methodological approach.en_US
