| dc.description.abstract | นโยบายระบบประกันสุขภาพผู้มีปัญหาสถานะและสิทธิ ได้รับการอนุมัติตามมติคณะรัฐมนตรี เมื่อวันที่ 23 มีนาคม 2553 มีวัตถุประสงค์เพื่อ (1) ให้บุคคลที่มีปัญหาสถานะและสิทธิสามารถเข้าถึงระบบบริการสุขภาพที่จำเป็น และ (2) เพื่อแก้ปัญหาภาระค่าใช้จ่ายของหน่วยบริการของกระทรวงสาธารณสุข และลดรายจ่ายจากการรับการรักษาพยาบาลของผู้ป่วย อย่างไรก็ตาม ตั้งแต่กำหนดนโยบายมา ยังไม่มีการประเมินผลจากการดำเนินงานตามนโยบายนี้อย่างเป็นระบบ โดยเฉพาะในเรื่องการบริหารจัดการและผลกระทบของนโยบายต่อสถานพยาบาล งานวิจัยฉบับนี้จึงจัดทำขึ้นเพื่อศึกษาและทำความเข้าใจการบริหารจัดการและการดำเนินการของสถานพยาบาลในการให้บริการสุขภาพ เพื่อให้บุคคลที่มีปัญหาสถานะและสิทธิได้เข้าถึงและได้รับการบริการสุขภาพที่จำเป็นภายใต้ระบบประกันสุขภาพผู้มีปัญหาสถานะและสิทธิ ตามมติคณะรัฐมนตรี 23 มีนาคม 2553 ผ่านกรณีศึกษาจังหวัดระนองและจังหวัดตาก การศึกษานี้พบว่าการบริหารจัดการระบบประกันสุขภาพผู้มีปัญหาสถานะและสิทธิตลอดสามปีที่ผ่านมา แม้จะประสบความสำเร็จในการลดค่าใช้จ่ายของผู้ป่วยที่มาใช้บริการสุขภาพ และเพิ่มรายได้ให้แก่สถานพยาบาล แต่ไม่ได้ส่งผลให้ผู้มีปัญหาสถานะและสิทธิเข้าถึงบริการสุขภาพมากขึ้น หรือเปลี่ยนแปลงภาระงานของสถานพยาบาล นอกจากนั้นยังมีข้อจำกัดและพบปัญหาในการบริหารจัดการระบบ ทั้งนี้ควรมีการบทบทวนข้อดี ข้อเสีย ของการบริหารจัดการระบบประกันสุขภาพผู้มีปัญหาสถานะและสิทธิในปัจจุบันเปรียบเทียบกับระบบบริหารจัดการอื่นๆ ร่วมด้วย | th_TH |

| dc.description.abstractalternative | This study aimed to better understand the management and the operation of health facilities under 'Health Insurance Policy for People with Citizenship Problems: the Cabinet Resolution on 23rd March 2010', which enabled the target population to access to essential health services, through a case study in Ranong and Tak province. This study suggests that HIS-PCP successfully reduced the expenditure from receiving healthcare services in the target population and also made health facilities gain more revenue. However, it did not signifcantly change the utilizations of the target population and the service burdens at health facilities. Futhermore, there remain unsolved problems in managing the system. A comprehensive review on advantages and disadvantages of the operation of HISPCP as status quo comparing with other alternatives in managing the system is recommended. | en_US |
