dc.description.abstractบุคลากรสาธารณสุขที่ปฏิบัติงานในโรงพยาบาลมีความเสี่ยงที่จะได้รับเชื้อวัณโรคขณะปฏิบัติงานสูงกว่าประชากรทั่วไป งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาสถานการณ์การป่วยเป็นวัณโรคของบุคลากรในโรงพยาบาลสังกัดกระทรวงสาธารณสุขที่อยู่ในจังหวัดตาก ระหว่างปีงบประมาณ 2551 - 2559 ผลการศึกษาพบว่า บุคลากรในโรงพยาบาลป่วยด้วยวัณโรค 46 ราย จากจำนวนบุคลากรเฉลี่ยต่อปี 2,956 คน คิดเป็นอัตราป่วย 15.6 ต่อประชากรพันคน เป็นเพศหญิงร้อยละ 69.6 มีอายุเฉลี่ย 33.6 ปี ลักษณะงานที่ปฏิบัติคือทำหน้าที่ดูแลผู้ป่วยเป็นหลักร้อยละ 71.7 ส่วนประเภทของวัณโรคนั้น พบป่วยด้วยวัณโรคปอดร้อยละ 69.5 หากจำแนกตามกลุ่มวิชาชีพ พบว่า แพทย์มีอัตราป่วยเป็นวัณโรคสูงที่สุด (38.5 ต่อประชากรพันคน) รองลงไป คือ เภสัชกร/เจ้าพนักงานเภสัชกรรม (35.1 ต่อประชากรพันคน) และพยาบาล (17.6 ต่อประชากรพันคน) ตามลำดับ เมื่อจำแนกตามสภาพพื้นที่ พบว่า อัตราป่วยด้วยวัณโรครวมทุกประเภทของบุคลากรที่ปฏิบัติงานในฝั่งตะวันตก 5 อำเภอชายแดนไทย-พม่าของจังหวัดตาก มีอัตราป่วย 17.7 ต่อประชากรพันคน สำหรับวัณโรคปอดพบว่าบุคลากรฝั่งตะวันตกมีอัตราป่วย 13.0 ต่อประชากรพันคน ส่วนฝั่งตะวันออกมีอัตราป่วย 6.7 ต่อประชากรพันคน ดังนั้นจึงจำเป็นที่จะต้องมีมาตรการคัดกรองผู้ป่วยที่สงสัยว่าจะเป็นวัณโรคที่อยู่ในชุมชนและให้การรักษาโดยเร็ว และรักษาให้หายขาด โดยเฉพาะในฝั่งตะวันตกของจังหวัดตาก รวมทั้งปรับปรุงการบริหารจัดการของโรงพยาบาลให้มีคุณภาพเพื่อคุ้มครองบุคลากรจากการป่วยด้วยวัณโรคth_TH

dc.description.abstractalternativeHealth personnel (HP) have higher risk of tuberculosis (TB) infection during working than people in the community. This research aims to determine TB situation among HP working in hospitals under the Ministry of Public Health, Tak Province, fiscal year 2008 - 2016. During the study period, of an average of 2,956 HP per year, 46 were registered for TB treatment, giving a rate of 15.6/1,000 HP. About 69.6% of them were female and the mean age was 33.6 years old. About 71.7% were personnel responsible mainly for patients’ management. Of all 46 TB cases, 69.5% had pulmonary TB. Overall, physicians had the highest TB rate (38.5/1,000), followed by pharmacists/pharmacy technicians (35.1/1,000) and nurses (17.6/1,000). Hospital personnel working in 5 districts (along Thailand-Myanmar border) of the western region of the province had a higher TB rate of all types (17.7/1,000), hospital personnel working in the western region had a higher TB rate of pulmonary TB (13.0/1,000) than those working in the eastern region (6.7/1,000). It is crucial to carry out early TB detection and treatment with high successful rates among people in the communities to reduce TB spread, particularly in the western region of Tak Province. In addition, good TB infection control should be continuously maintained in the hospitals.en_EN
