dc.description.abstractประเทศไทยบรรลุระบบหลักประกันสุขภาพถ้วนหน้า ตั้งแต่ พ.ศ. 2545 ภายใต้ระบบประกันสุขภาพภาครัฐสามระบบหลัก ทำให้ประชาชนสามารถเข้าถึงบริการสุขภาพที่จำเป็นได้มากขึ้น โดยไม่ต้องกังวลเรื่องภาระค่าใช้จ่ายสำหรับการรักษาพยาบาล แต่ก็ยังคงมีความจำเป็นทางสุขภาพที่ไม่ได้รับการตอบสนอง (unmet health need) จึงได้มีการศึกษา unmet health need ซึ่งเป็นการติดตามผลการดำเนินงานหลักประกันสุขภาพถ้วนหน้าในอีกมุมหนึ่ง ที่จะทำให้ได้ทราบสถานการณ์ว่ามีการเข้าไม่ถึงบริการสุขภาพหรือไม่ มากน้อยอย่างไร และด้วยเหตุผลใด การศึกษานี้มีวัตถุประสงค์เพื่อวิเคราะห์ความชุกของ unmet health need ลักษณะประชากร ปัจจัยที่มีผล และเหตุผลที่ทำให้เกิด unmet health need ทั้งกรณีผู้ป่วยนอก ผู้ป่วยใน และบริการทันตกรรม โดยใช้สถิติเชิงพรรณนาและแบบจำลองโลจิต วิเคราะห์ข้อมูลจากการสำรวจอนามัยและสวัสดิการ พ.ศ. 2558 มีตัวอย่างทั้งสิ้น 55,920 ครัวเรือนที่เป็นตัวแทนประชากรไทย ผลการศึกษาพบว่า ความชุกของ unmet health need กรณีผู้ป่วยนอก ผู้ป่วยในและบริการทันตกรรม เท่ากับร้อยละ 1.50, 0.14 และ 0.99 ของประชากรทั้งหมดตามลำดับ ปัจจัยที่มีผลต่อการเกิด unmet health need อย่างมีนัยสำคัญ คือ อายุ โดยเฉพาะผู้สูงอายุ 60 ปีขึ้นไป มีโอกาสเกิด unmet health need มากกว่าผู้ที่มีอายุน้อยกว่า การไม่มีสิทธิในระบบประกันสุขภาพมีโอกาสเกิด unmet health need มากกว่าผู้ที่มีสิทธิประกันสุขภาพ เศรษฐฐานะครัวเรือนกลุ่มจนที่สุดมีโอกาสเกิด unmet health need มากกว่ากลุ่มที่มีเศรษฐฐานะดีกว่า และผู้ที่อาศัยในกรุงเทพมหานครมีโอกาสเกิด unmet health need กรณีผู้ป่วยนอกมากกว่าผู้ที่อาศัยในภาคอื่นๆ เหตุผลส่วนใหญ่ที่ทำให้เกิด unmet health need คือ คิวยาว/รอนาน ไม่มีเวลาไปรับการรักษา และเดินทางไม่สะดวก/อยู่ห่างไกล ทั้งกรณีผู้ป่วยนอก ผู้ป่วยในและบริการทันตกรรม กระทรวงสาธารณสุข สำนักงานหลักประกันสุขภาพแห่งชาติ และหน่วยงานอื่นๆ ที่เกี่ยวข้องควรเร่งหาแนวทางหรือจัดทำแผนยุทธศาสตร์ในการดูแลผู้สูงอายุและประชาชนทุกคนให้เข้าถึงระบบบริการสุขภาพที่จำเป็นอย่างยั่งยืน รวมถึงการพัฒนาระบบบริการสุขภาพปฐมภูมิให้ครอบคลุมทุกพื้นที่ในเขตเมือง โดยเฉพาะอย่างยิ่งเขตกรุงเทพมหานคร และการจัดให้มีสถานพยาบาลใกล้บ้านใกล้ใจให้ครอบคลุมทุกพื้นที่th_TH

dc.description.abstractalternativeSince 2002, Thailand has achieved Universal Health Coverage (UHC) under the three main public health insurance schemes. UHC enabled Thai population to access essential healthcare services regardless of income. Patient would not be worried about negative financial impacts from medical bills to their household however the unmet health need can be found. Therefore, there is a study on unmet health need conducted in order to monitor UHC on another aspect and to demonstrate whether or not Thailand has patients who cannot access health service when needed, if yes, how many; who are they and why? The objective of this study is to analyze prevalence of unmet health need, to describe profile of patients with unmet health need and to identify factors and reasons for unmet health need. Descriptive statistics and logit model were applied for analysis of Health and Welfare Survey (HWS) in 2015 with 55,920 sampled households representing Thai populations. Prevalence of unmet health need for outpatient, inpatient and dental service were 1.50 percent, 0.14 percent and 0.99 percent of all population, respectively. Main factors of being unmet health need were age, especially elderly; uninsured people; and the poorest household economic group. In addition, people living in Bangkok had more unmet health need for outpatient services than other regions. The main reasons of unmet health need were inadequate time of patient for seeking care, too long queue at health facilities, and too far to travel to health facilities. Ministry of Public Health, National Health Security Office and other relevant agencies should collectively work together for a policy and strategy to reduce unmet health need of Thai population, especially for elderly. Primary health care service delivery system should be strengthened to cover all urban areas in particular in Bangkok. “Close to client health services” should be developed for improving access to health services.en_EN
