Health promotion and disease prevention for all at all ages relies on the participation of local communities. Nan province has strong and various social groups, organizes and health networks, but many health outcomes are not achieved. Therefore, the aims of this study were toexamine the development of the civil society in health promotion, to study the situation and the condition of the sustainability of health groups / organizations, to determine factors that affect the success and outcome of policy implementation and the participation of the civil society, and to synthesize proposals for the development of community health systems. Implementation research were conducted in three phases: 1) study of situational, group and health networking, 2) operations and 3) assessment, between April 2516 and December 2517. The quantitative data was analyzed using frequency and percentage, and the qualitative data was analyzed using content analysis. The study found that Nan has a good cost of health care, with the involvement of network partners in all of study area. The civil society were formed through three different processes: 1) to start with governmental or private policy from the outside, 2) to start from the organization within the area, 3) to initiate from the community organization itself. Factors of maintaining the strength of the civil society include strong leadership, faithful acceptance, transparency, multi-level networking, informal and formal coordination, having regular meetings, continuously develop the potential, running and managing the welfare fund, set up community or district health measures, and having a review summary evaluation together. The development of the civil society begins with the initiation of people in the community who need development or government policies. The process start with aggregate review of potential, limitations, goals, common development of community health care. Next, they develop a plan to improve the existing learning process. Then they build the learning process to develop the potential for leaders and volunteer. In addition, they raise the strength of the management with repeatedly accumulate knowledge. Finally, they achieve the best practices for community health carethat isled to the strength of the health community. Factors influencing the success and outcome of policy implementation are as follows : building central concept, creating an organizational structure with a clear roles, raising community awareness, Based on the results, we suggest that health promotion and disease prevention should be initiated by the public health volunteers and the elderly. They are a core group that plays an important role in the development of health and disease prevention in all age groups. Furthermore, toward self-reliance and sustainable community, the role of the health workers is to act as the coach of community health and networks by using an empowered communities approach. building strong leader, enhancing community cooperation, empowering and learning with the young people's community worker, working with collaborating networks, using informal communication, learning by using case studies and being put into practice, actively involving diverse volunteers in community, making a variety of communication channels, particularly, Facebook and Line applications, continuously supporting by stakeholders, and making a participatory monitoring and evaluation. Based on the results, we suggest that health promotion and disease prevention should be initiated by the public health volunteers and the elderly. They are a core group that plays an important role in the development of health and disease prevention in all age groups. Furthermore, toward self-reliance and sustainable community, the role of the health workers is to act as the coach of community health and networks by using an empowered communities approach.
