dc.description.abstractโรคติดเชื้อไวรัสโคโรนา 2019 (COVID-19) เริ่มมีการระบาดครั้งแรกในเดือนธันวาคม 2562 ณ ประเทศจีน หลังจากนั้นมีการแพร่ระบาดเป็นวงกว้างทั่วโลก รวมถึงประเทศไทย รัฐบาลไทยได้ประกาศพระราชกำหนดการบริหารราชการในสถานการณ์ฉุกเฉิน พ.ศ. 2548 มีผลบังคับใช้ตั้งแต่ วันที่ 26 มีนาคม 2563 เป็นต้นไป และราชกิจจานุเบกษาเผยแพร่ประกาศห้ามบุคคลใดทั่วราชอาณาจักรออกนอกเคหสถาน ระหว่างเวลา 22.00 น. ถึง 04.00 น. เริ่มวันที่ 3 เมษายน 2563 ซึ่งอาจจะมีผลกระทบต่องานด้านวัณโรค โครงการวิจัยนี้มีวัตถุประสงค์หลัก คือ เพื่อเปรียบเทียบผลการดำเนินงานควบคุมวัณโรคในประเทศไทย ก่อน-หลังการระบาดโรคติดเชื้อไวรัสโคโรนา 2019 วัตถุประสงค์รอง คือ 1. เพื่อศึกษาผลการวินิจฉัยวัณโรคทางห้องปฏิบัติการ 2. เพื่อศึกษาผลการขึ้นทะเบียนผู้ป่วยวัณโรคประเภทต่างๆ และ 3. เพื่อศึกษาผลการรักษาผู้ป่วยวัณโรคประเภทกลุ่มต่างๆ การศึกษาวิจัยครั้งนี้ เป็นการวิจัยสำรวจภาคตัดขวาง (cross-sectional study) โดยใช้ข้อมูลทุติยภูมิ (Secondary Data) ที่มีการขึ้นทะเบียนผู้ป่วยวัณโรคในโปรแกรม National Tuberculosis Information Program (NTIP) นำมาวิเคราะห์ในช่วงเวลา ไตรมาสที่ 1/2562 และ 1/2563 (สำหรับการศึกษาประเภทของผลการรักษาวัณโรค เป็นก่อน-หลังเริ่มมีการระบาดระลอกแรกเมื่อไตรมาสที่ 1 ของปีงบประมาณ 2562 และ 2563 เปรียบเทียบคนละปี) และในช่วงเวลาไตรมาสที่ 2-3/2562 และ 2-3/2563 ก่อน-หลังเริ่มมีการระบาดระลอกแรกเมื่อปีงบประมาณ 2562 และ 2563 เปรียบเทียบคนละปี (สำหรับการศึกษาประเภทของผลการวินิจฉัยเป็นวัณโรค) ผลการศึกษา พบว่า การขึ้นทะเบียนของผู้ป่วยวัณโรคทุกประเภท ก่อน-หลังเริ่มมีการระบาดระลอกแรกเมื่อไตรมาสที่ 2-3 ของปีงบประมาณ 2562 และ 2563 เปรียบเทียบคนละปี มีจำนวน 40,544 และ 40,848 ราย ตามลำดับ เพิ่มขึ้นโดยไม่มีนัยสำคัญทางสถิติ (p=0.275) การขึ้นทะเบียนของผู้ป่วยประเภทรายใหม่และกลับเป็นซ้ำ ก่อน-หลังเริ่มมีการระบาดระลอกแรกเมื่อไตรมาสที่ 2-3 ของปีงบประมาณ 2562 และ 2563 เปรียบเทียบคนละปี คิดเป็นจำนวน 38,875 และ 39,231 ราย ตามลำดับ เพิ่มขึ้นโดยไม่มีนัยสำคัญทางสถิติ (p=1.00) หากเปรียบเทียบก่อน-หลังการประกาศ lockdown (ระหว่างไตรมาสที่ 2/63 และ 3/63) เปรียบเทียบปีเดียวกัน มีจำนวน 21,358 และ 17,873 ราย ตามลำดับ ลดลง -16.3% จำนวนการส่งตรวจ AFB อย่างเดียว จำนวน 23,792 (61.2%) และ 25,731 (65.6%) ราย ตามลำดับ เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p<0.001) แต่การส่งตรวจเสมหะด้วยเครื่อง Xpert MTB/RIF อย่างเดียว จำนวน 717 (1.8%) และ 577 (1.5%) ราย ตามลำดับ พบว่าลดลงอย่างมีนัยสำคัญทางสถิติ (p<0.001) สำหรับจำนวนผู้ป่วยที่ส่งตรวจทั้ง AFB และวิธี Xpert MTB/RIF โดย AFB ส่งก่อนถ้าผลเป็นลบแล้วจึงส่ง Xpert MTB/RIF จำนวน 8,838 (22.7%) และ 6,808 (17.4%) ราย ตามลำดับ ลดลงอย่างมีนัยสำคัญทางสถิติ (p<0.001) จำนวนผู้ป่วยที่ไม่มีผลการตรวจ มีจำนวน 5,528 (14.2%) และ 6,115 (15.6%) ราย ตามลำดับ เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p<0.001) ผลการรักษาวัณโรคในกลุ่มผู้ป่วยรายใหม่และกลับเป็นซ้ำ พบว่า อัตราผลสำเร็จของการรักษาผู้ป่วยวัณโรค (Treatment success rate) ก่อน-หลังเริ่มมีการระบาดระลอกแรกเมื่อไตรมาสที่ 1 ของปีงบประมาณ 2562 และ 2563 เปรียบเทียบคนละปี คิดเป็น 80.8% และ 86.3% ตามลำดับ ลดลงอย่างมีนัยสำคัญทางสถิติ (p<0.001) อัตราการโอนออก คิดเป็น 0.7% และ 1.3% ตามลำดับ เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p<0.001) และผู้ป่วยที่กำลังรักษา ก่อน-หลังเริ่มมีการระบาดระลอกแรกเมื่อไตรมาสที่ 1 ของปีงบประมาณ 2562 และ 2563 เปรียบเทียบคนละปี คิดเป็น 0.8% และ 6.2% ตามลำดับ เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p<0.001) สำหรับอัตราการขาดยา คิดเป็น 4.3% และ 3.7% ตามลำดับ ลดลงอย่างมีนัยสำคัญทางสถิติ (p=0.016) การรักษาล้มเหลว คิดเป็น 0.3% และ 0.3% ตามลำดับ โดยไม่มีนัยสำคัญทางสถิติ (p=1.00) และการเสียชีวิต คิดเป็น 7.5% และ 7.7% ตามลำดับ เพิ่มขึ้นอย่างไม่มีนัยสำคัญทางสถิติ (p=1.00) สรุป การศึกษานี้เป็นการศึกษาแรกในระดับประเทศ ที่ศึกษาผลกระทบของสถานการณ์การระบาดของโรคติดเชื้อไวรัสโคโรนา 2019 ทำให้มีหลักฐานเชิงประจักษ์ว่า การขึ้นทะเบียนผู้ป่วยวัณโรคลดลง อัตราผลสำเร็จของการรักษาลดลง การรายงานจำนวนผู้ป่วยวัณโรคทุกประเภทและกลุ่มรายใหม่และกลับเป็นซ้ำ มีจำนวนการขึ้นทะเบียนไม่แตกต่างกัน แต่ในไตรมาสหลังมีแนวโน้มลดลง โดยเฉพาะช่วงที่มีการประกาศ lockdown จำนวนการวินิจฉัยด้วย Xpert MTB/RIF ลดลง รวมถึงร้อยละของผลสำเร็จในการรักษาที่ลดลง เมื่อเปรียบเทียบก่อน-หลังเริ่มมีการระบาดระลอกแรกเมื่อไตรมาสที่ 1 ของปีงบประมาณ 2562 และ 2563 เปรียบเทียบคนละปี เพื่อให้การดำเนินงานเป็นไปอย่างมีประสิทธิภาพในช่วงการแพร่ระบาดโรค COVID-19 ควรเร่งรัดการค้นหาผู้ป่วยวัณโรคให้ครอบคลุมโดยการคัดกรองในกลุ่มเสี่ยงต่างๆ เช่น กลุ่มผู้ที่มาใช้บริการที่โรงพยาบาล เพิ่มการค้นหากลุ่มเสี่ยงในชุมชนหรือสถานกักกัน เพื่อเข้าสู่การรักษาได้อย่างรวดเร็ว โดยอาจจะคัดกรองควบคู่ไปกับการคัดกรองโรค COVID-19 ผลการศึกษานี้ยังเป็นข้อมูลในการเตรียมความพร้อมทางด้านการขึ้นทะเบียนการรักษา การตรวจเพื่อการวินิจฉัยและผลการรักษาวัณโรค หากเกิดการระบาดของระลอกต่อๆ ไปของโรคติดเชื้อไวรัสโคโรนา 2019th_TH

dc.description.abstractalternativeSince the novel coronavirus outbreak (COVID-19) was identified for the first time in China during December, 2019. The world faces the COVID-19 epidemic including Thailand. The Royal Thai Government has announced an Emergency Decree on Public Administration in an Emergency Situation B.E. 2548 which had been effected from 26 March 2020 onwards. The Royal Thai Government Gazette published an announcement prohibiting any person throughout the Kingdom from leaving their residence between 10:00 PM and 4:00 AM starting on April 3, 2020. It may impact national tuberculosis program. The main objective of this study was to compare the TB treatment outcomes during pre-and post-COVID-19 outbreak. The secondary objectives were 1. To evaluate the number of TB diagnosis test during pre- and post-COVID-19 outbreak, 2. To compare TB case notification during pre- and post-COVID-19 outbreak, and 3. To compare TB treatment outcomes during pre and post COVID-19 outbreak. A cross-sectional study was conducted in this research by using the secondary data which TB patients were registered into the NTIP program. Comparing the data of the first cohort of the fiscal year 2019 and the first cohort of the fiscal year 2020 for the topic of TB treatment outcomes pre- and post- of the first wave of outbreaks in 2019 and 2020. TB diagnosis analysis we used the data of the two cohorts (second and third cohort of the fiscal year 2019) and the data of two cohorts (second and third cohort of the fiscal year 2020) representing pre-and post- of the first wave of outbreaks in 2019 and 2020. The results of this study showed that the number of all TB cases during pre- and post- of the first wave of COVID-19 outbreak in 2019 and 2020 were 40,544 and 40,848 cases, respectively with slightly increase but not statistically different (p= 0.275). The case notification number of new and relapse cases were 38,875 and 39,231 cases, respectively with an increase but not statistically different (p=1.00). Before and after the lockdown (between the same year between cohort Q2/2020 and 3/20), there were 21,358 and 17,873 cases, respectively, a decrease of -16.3%. The number of TB patients who diagnosed by AFB smear of 23,792 (61.2%) and 25,731 (65.6%) cases, respectively showed a statistically significant increase (p<0.001). In contrast, the number of TB patients who diagnosed by only Xpert MTB/RIF of 717 (1.8%) and 577 (1.5%) cases, respectively showed a statistically significant reduction (p<0.001). In addition, during pre- and post- of the first wave of outbreaks in 2019 and 2020, the number of patients diagnosed by both AFB and Xpert MTB/RIF (AFB smear is initiated test for TB diagnosis, if AFB negative was founded the Xpert MTB/RIF is done) wirh 8,838 (22.7%) and 6,808 (17.4%), respectively showed a statistically significant reduction (p<0.001). The number of TB patients without results were 5,528 (14.2%) and 6,115 (15.6%), respectively, a statistically significant increase (p<0.001). The TB treatment outcomes of new and relapse cases during pre-and post- of the first wave of COVID-19 outbreaks in 2019 and 2020 showed that the success rate were 80.8% and 86.4%, respectively. There was a statistically significant decrease (p<0.001). Transferred-out were 0.7% and 1.3% with a statistically significant increase (p<0.001). Lost-to-follow up rate were 4.3% and 3.7%, respectively with a statistically significant reduction (p<0.001). On-treatment rate accounted for 0.8% and 6.2% with a statistically significant increase (p<0.001). In summary, this study is the first national study that presented the evidence of impact of the Covid-19 outbreak situation to TB programmes and services. Case notification number among new and relapse cases during post of the first wave of COVID-19 outbreak was lower, especially lockdown period, but not statistically different. The number of diagnosis examinations by Xpert MTB/RIF was declined. Moreover, treatment success rate trended to reduction. In order to operate TB programme effectively during the COVID-19 epidemic, an intensive TB cases finding especially among the high risk groups, for examples; out-patients and the prisoners should be done as well as screen COVID-19 in the same time. The results of this study provided the useful information of the trend of case notification, TB diagnosis test and TB treatment outcomes for prepare the TB strategies for the next wave of COVID-19 outbreaks.th_TH
