dc.description.abstractงานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาต้นทุนรูปแบบบริการสุขภาพ (health service models) ผู้ป่วยโควิด-19 ที่โรงพยาบาลจัดไว้สำหรับรองรับการรักษาตามระดับอาการของผู้ป่วย โดยวิเคราะห์ต้นทุนในมุมมองผู้ให้บริการ ใช้การวิเคราะห์ต้นทุนบริการแบบมาตรฐาน (standard costing method) และวิเคราะห์ต้นทุนรายผู้ป่วยด้วยวิธีต้นทุนจุลภาคหรือจากล่างขึ้นบน (micro-costing or bottom-up approach) ใช้ข้อมูลจากโรงพยาบาลศูนย์ 3 แห่ง และโรงพยาบาลทั่วไป 2 แห่ง รวม 5 แห่ง ศึกษาต้นทุนปีงบประมาณ 2564 (ต.ค. 2563 - ก.ย. 2564) โดยคัดเลือกข้อมูลเฉพาะผู้ป่วยในที่เป็นโควิด-19 จากรหัสโรคที่โรงพยาบาลบันทึกไว้ รูปแบบบริการสุขภาพในโควิด-19 ในการศึกษานี้มี 7 รูปแบบ ได้แก่ 1) หอผู้ป่วยโควิด-19 (cohort ward) 2) หอผู้ป่วยวิกฤตโควิด-19 (intensive care unit-cohort ward 3) หอผู้ป่วยเฉพาะกิจ (hospitel) 4) โรงพยาบาลสนาม (field hospital) 5) โรงพยาบาลสนามในเรือนจำ (field hospital for prisoners) 6) กักตัวที่บ้าน (home isolation) และ 7) กักตัวในชุมชน (community isolation) ผลการศึกษา ต้นทุนการรักษาผู้ป่วยในโควิด-19 เฉลี่ย 31,342 บาทต่อราย วันนอนเฉลี่ย 10.9 วันต่อราย ต้นทุนผู้ป่วยที่หอผู้ป่วยโควิด-19 62,033 บาทต่อราย ต้นทุนผู้ป่วยที่หอผู้ป่วยวิกฤตโควิด-19 187,257 บาทต่อราย ต้นทุนผู้ป่วยที่หอผู้ป่วยเฉพาะกิจ 18,125 บาทต่อราย ต้นทุนผู้ป่วยที่โรงพยาบาลสนาม 19,589 บาทต่อราย ต้นทุนผู้ป่วยที่โรงพยาบาลสนามในเรือนจำ 24,318 บาทต่อราย ต้นทุนผู้ป่วยที่กักตัวที่บ้าน 6,039 บาทต่อราย ต้นทุนผู้ป่วยที่กักตัวในชุมชน 4,532 บาทต่อราย ต้นทุนของผู้ป่วยในโควิด-19 ที่มีการเปลี่ยนรูปแบบบริการในระหว่างการรักษาจากนอกโรงพยาบาลย้ายเข้ารักษาภายในโรงพยาบาล มีต้นทุนรวมเฉลี่ย 91,568 บาทต่อราย วันนอนรวมเฉลี่ย 11.0 วัน และต้นทุนผู้ป่วยที่เปลี่ยนรูปแบบบริการอยู่ภายในโรงพยาบาล (cohort ward ไป ICU-cohort ward) ต้นทุนรวมเฉลี่ย 149,844 บาทต่อราย วันนอนรวมเฉลี่ย 12.1 วัน ข้อเสนอแนะ งานวิจัยนี้ สามารถสะท้อนให้เห็นถึงต้นทุนการใช้ทรัพยากรในแต่ละรูปแบบบริการของผู้ป่วยในโควิด-19 ซึ่งมีความแตกต่างกัน หากศึกษาเพิ่มเติมเกี่ยวกับจำนวนงบประมาณ ที่กองทุนประกันสุขภาพจ่ายชดเชยค่าบริการให้กับโรงพยาบาล เปรียบเทียบกับต้นทุนที่แท้จริง จะสามารถสะท้อนความพอเพียงของงบประมาณ และวิธีการจ่ายเงินของกองทุนว่าเหมาะสมสอดคล้องกับการใช้ทรัพยากรของโรงพยาบาลเพียงใดth_TH

dc.description.abstractalternativeThis research aimed to study the cost of health service model for COVID-19 inpatient care that hospitals provided according to the level of inpatient’s condition. The data were from a 5-hospitals case study in Thailand, employing a standardized top-down cost analysis and the micro-costing method based on provider perspective. Three regional and two general hospitals participated by providing hospital data of the fiscal year 2021. There were 7 health service models under study: 1) cohort ward 2) intensive care unit-cohort ward 3) hospitel 4) field hospital 5) field hospital for prisoners 6) home isolation and 7) community isolation. The average cost of treating COVID-19 patients was 31,342 baht per case for the average length of stay (aLOS) of 10.9 days. The cost at the ICU-cohort ward was the highest at 187,257 baht per case, followed by the cost at the cohort ward at 62,033 baht per case and the lowest cost at community isolation at 4,532 baht per case. For COVID-19 patients who experienced more than one service models due to clinical condition change, the average total cost was 91,568 baht per case for aLOS of 11.0 days for those cared in community then moved to hospital; and 149,844 baht per case with aLOS of 12.1 days for those cared in cohort ward then moved to ICU-cohort ward. The results of this study demonstrating different resource uses by service models could be used to evaluate the amount of COVID-19 budgets that health insurance funds made available and paid for service delivery of the hospital compared to the actual cost by service model.th_TH
