dc.description.abstractการตรวจวินิจฉัยวัณโรคระยะแฝงและการจัดการที่เหมาะสม เป็นองค์ประกอบสำคัญในการลดจำนวนผู้ป่วยและผู้เสียชีวิตจากวัณโรค การตรวจวินิจฉัยวัณโรคระยะแฝง ด้วยการตรวจสารอินเตอร์เฟอรอนแกมมาจากตัวอย่างเลือด มีข้อดีกว่าการทดสอบทางผิวหนังด้วยน้ำยาทุเบอร์คุลิน แต่ชุดตรวจที่ใช้งานในปัจจุบันมีราคาสูงและต้องการการทดสอบด้วยวิธี Enzyme-Linked Immunosorbent Assay (ELISA) ที่มีความซับซ้อนและไม่เหมาะสมสำหรับห้องปฏิบัติการระดับอำเภอหรือจังหวัด ชุดตรวจ QIAreach QuantiFERON-TB (QIAREACH-TB) เป็นการตรวจสารอินเตอร์เฟอรอนแกมมาจากตัวอย่างเลือดแบบใหม่ ด้วยเทคนิค Lateral Flow Immunoassay (LFA) ร่วมกับการวัดสัญญาณฟลูออเรสเซนต์แบบดิจิทัล การศึกษานี้มีวัตถุประสงค์ เพื่อประเมินชุดตรวจวินิจฉัยวัณโรคระยะแฝงจากตัวอย่างเลือดแบบใหม่ QIAREACH-TB เปรียบเทียบกับชุดตรวจแบบเดิม QuantiFERON-TB Gold Plus (QFT-Plus) โดยวิธีการศึกษา คือ เก็บตัวอย่างเลือดจากอาสาสมัครกลุ่มบุคลากรทางการแพทย์ จำนวน 154 ราย และตรวจวินิจฉัยวัณโรคระยะแฝงด้วยชุดตรวจ QFT-Plus และ QIAREACH-TB ผลการศึกษา เมื่อเปรียบเทียบกับชุดตรวจ QFT-Plus ชุดตรวจ QIAREACH-TB มีค่า Overall Percentage Agreement; OPA (หรือค่าความถูกต้อง) เท่ากับร้อยละ 93.46 (ช่วงความเชื่อมั่นร้อยละ 95 [95% Confidence Interval; 95% CI]: 88.31-96.82) ค่า Positive Percent Agreement; PPA (หรือค่าความไว) เท่ากับร้อยละ 88.46 (95% CI: 69.85-97.55) และค่า Negative Percent Agreement; NPA (หรือค่าความจำเพาะ) เท่ากับร้อยละ 94.49 (95% CI: 88.97-97.76) โดยสรุป ชุดตรวจ QIAREACH-TB มีผลการประเมินความสอดคล้องกับชุดตรวจ QFT-Plus แบบเดิมในระดับสูง แต่ใช้โครงสร้างและทรัพยากรทางห้องปฏิบัติการน้อยกว่า เนื่องจากไม่ต้องใช้การทดสอบด้วยวิธี ELISA ส่งผลให้สามารถขยายมาตรการการค้นหาผู้ติดเชื้อวัณโรคระยะแฝง ตามแผนยุทธศาสตร์แผนปฏิบัติการระดับชาติด้านการต่อต้านวัณโรคต่อไปth_TH

dc.description.abstractalternativeDiagnosis and proper management of latent tuberculosis infection (LTBI) are important components in reducing the number of patients and deaths from tuberculosis (TB). The diagnosis of LTBI using the interferon-gamma release assays (IGRAs) to detect interferon-gamma in bloods sample has advantages over the tuberculin skin test (TST), but the currently used test kits are expensive and require enzyme-linked immunosorbent assay (ELISA), making it complicate and unsuitable for a district or provincial laboratory use. The QIAreach QuantiFERON-TB (QIAREACH-TB) is a novel blood-based test that uses lateral flow immunoassay (LFA) and digital fluorescence signal measurement to detect interferon-gamma. This study aimed to evaluate the QIAREACH-TB for the diagnosis of LTBI using a whole-blood sample compared to the QuantiFERON-TB Gold Plus (QFT-Plus). Blood samples were collected from 154 healthcare workers (HCWs) and tested for LTBI using the QFT-Plus and QIAREACH-TB. The results showed that the overall percentage agreement (OPA; accuracy) between the QIAREACH-TB and QFT-Plus was 93.46% (95% confidence interval [95% CI]: 88.31-96.82), the positive percent agreement (PPA; sensitivity) was 88.46% (95% CI: 69.85-97.55), and the negative percent agreement (NPA; specificity) was 94.49% (95% CI: 88.97-97.76). In conclusion, the QIAREACH-TB has high agreements with the QFT-Plus, but requires fewer laboratory resources and does not require ELISA, thus making it allows for the expansion of efforts to detect individuals with LTBI following the national TB control strategy.th_TH
