การวิจัยและพัฒนาเครื่องมือรายงานโดยผู้ป่วย (Patient Report Outcomes Measurement) เพื่อใช้ในการพัฒนาระบบบริการแบบเน้นคุณค่าสำหรับผู้ป่วยเบาหวาน

บทคัดย่อ
การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อปรับปรุงเครื่องมือประเมินผลลัพธ์การรักษาที่ได้จากการรายงานของผู้ป่วย สำหรับผู้ป่วยเบาหวานในประเทศไทยที่สอดคล้องกับวิถีชีวิตและวิธีการทำงานระดับปฏิบัติการของพื้นที่ต่างๆ และเพื่อพัฒนา Patient’s Journey ชุดบริการมาตรฐาน (Service Package) ที่รองรับการนำ Patient-Reported Outcome Measures: PROMs ไปใช้จริงในพื้นที่ การวิจัยใช้รูปแบบการวิจัยและพัฒนา (Research and Development) ร่วมกับการวิจัยเชิงดำเนินการ (Implementation Research) ดำเนินการในพื้นที่นำร่อง 3 แห่ง ได้แก่ อำเภอท่าวุ้ง จังหวัดลพบุรี อำเภอเมือง จังหวัดเพชรบูรณ์ และอำเภอละงู จังหวัดสตูล แบ่งเป็น 2 ระยะ ได้แก่ ระยะที่ 1 การพัฒนาและตรวจสอบคุณภาพเครื่องมือ PROMs โดยปรับปรุงจากต้นแบบ PRO-DM-Thai และพัฒนาเป็นทั้งฉบับพื้นที่และฉบับมาตรฐาน (Central PROMs Version) พร้อมตรวจสอบความตรงเชิงเนื้อหา ความตรงเชิงโครงสร้าง และความเชื่อมั่นของเครื่องมือ และระยะที่ 2 การนำ PROMs ไปใช้โดยทำการออกแบบ Patient’s Journey ด้วยการวิเคราะห์หาจุดแข็งจุดอ่อนของพื้นที่ทั้ง 3 แห่ง ซึ่งพบประเด็นสำคัญในการนำไปใช้ในบริบทจริง (In The Real World) คือ การใช้เพื่อสร้างประโยชน์ให้บุคลากรเห็นอย่างรวดเร็วและช่วยเพิ่มคุณค่าให้กับบริการแต่ละจุดได้จริง จึงทำให้เกิดการยอมรับ ซึ่งทีมวิจัยได้ทำการพัฒนาแบบ Co-Creation ร่วมกับพื้นที่ทั้ง 3 แห่ง เกิดรูปแบบโครงสร้างแบบแยกส่วน (Modular PROMs) ที่ทำให้เหมาะสมกับจุดบริการ กระจายภาระงาน สร้างการยอมรับได้ดี แล้วจึงได้ทดลองนำร่องเป็นระยะเวลา 1 เดือน กลุ่มตัวอย่างประกอบด้วยผู้ป่วยโรคเบาหวานชนิดที่ 2 จำนวน 180 คน แบ่งเป็นกลุ่มตรวจสอบคุณภาพเครื่องมือ (n=90) และกลุ่มทดลองนำร่อง (n=90) เก็บข้อมูลเชิงปริมาณและเชิงคุณภาพ โดยใช้แบบสอบถาม การสังเกต การสัมภาษณ์ และการสนทนากลุ่ม วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสถิติเชิงอนุมาน ได้แก่ ค่าสัมประสิทธิ์สหสัมพันธ์แบบสเปียร์แมน และค่าสัมประสิทธิ์ weighted kappa ผลการศึกษาพบว่า เครื่องมือ PROMs มีความเหมาะสมกับบริบทพื้นที่ และมีคุณภาพอยู่ในระดับที่ยอมรับได้ในด้านความตรงและความเชื่อมั่น ในระยะทดลองนำร่อง พบว่า การนำ PROMs ไปใช้สามารถดำเนินการได้จริงในหน่วยบริการปฐมภูมิ โดยเฉพาะเมื่อมีการปรับกระบวนการให้สอดคล้องกับบริบทพื้นที่โดยคำนึงถึงภาระงานและประโยชน์ที่ผู้ปฏิบัติได้ประสบในทันทีที่ใช้ PROMs โดยเฉพาะการเป็นเครื่องมือเปิดบทสนทนาที่ช่วยกระตุ้นให้ผู้ป่วยได้ระลึก (Awareness) ถึงความสำคัญของการจัดการดูแลตนเองและช่วยแยกอาการที่สำคัญได้ในเวลาอันสั้น ทั้งนี้ ผลลัพธ์ด้านการนำไปใช้สะท้อนถึงระดับการยอมรับ ความเป็นไปได้ การนำไปใช้จริง ความเหมาะสม และความสอดคล้องกับรูปแบบการดำเนินงานในระดับที่น่าพอใจ อย่างไรก็ตาม ระยะเวลาการทดลองที่จำกัดยังไม่สามารถสะท้อนผลลัพธ์ทางสุขภาพในระยะยาวได้ ข้อค้นพบจากการศึกษานี้ชี้ให้เห็นว่า PROMs สามารถทำหน้าที่เป็นเครื่องมือสำคัญในการเชื่อมโยงข้อมูลจากผู้ป่วยเข้าสู่การตัดสินใจเชิงบริการ และสร้างประสบการณ์ใหม่ให้กับทั้งบุคลากรและผู้ป่วย แสดงถึงศักยภาพในการสนับสนุนการพัฒนาระบบบริการสุขภาพแบบมุ่งเน้นคุณค่า (Value-Based Health Care: VBHC) ทั้งนี้ การขยายผลควรให้ความสำคัญกับการออกแบบระบบที่ยืดหยุ่น การพัฒนาศักยภาพบุคลากร และการติดตามผลลัพธ์ระยะยาว

บทคัดย่อ
The objectives of this study were to adapt and improve Patient-Reported Outcome Measures (PROMs) for diabetes patients in Thailand so that they align with local lifestyles and the operational workflows of different service settings, and to develop a Patient’s Journey and a standardized Service Package to support the practical implementation of PROMs in real-world settings. This study employed a Research and Development (R&D) design combined with Implementation Research. The study was conducted in three pilot sites: Tha Wung District, Lopburi Province; Mueang District, Phetchabun Province; and La-ngu District, Satun Province. The research was carried out in two phases. Phase 1 involved the development and psychometric evaluation of the PROMs instrument. The instrument was adapted from the PRO-DM-Thai prototype and further developed into both site-specific versions and a standardized version (Central PROMs Version). The instrument was evaluated for content validity, construct validity, and reliability. Phase 2 focused on implementing PROMs through the design of a Patient’s Journey based on an analysis of the strengths and weaknesses of the three pilot areas. The findings highlighted key factors for real-world implementation, particularly the importance of demonstrating immediate benefits to healthcare providers and adding value to service delivery at each touchpoint, which facilitated acceptance. The research team collaboratively co-created with the three pilot sites to develop a Modular PROMs structure tailored to service points, capable of distributing workload and enhancing acceptance. The model was then piloted for one month. The sample consisted of 180 patients with Type 2 Diabetes Mellitus, including 90 participants in the psychometric testing phase and 90 participants in the pilot implementation phase. Both quantitative and qualitative data were collected using questionnaires, observations, interviews, and focus group discussions. Data were analyzed using descriptive statistics and inferential statistics, including Spearman’s rank correlation coefficient and weighted kappa coefficient. The findings indicated that the PROMs instrument was appropriate for the local context and demonstrated acceptable validity and reliability. During the pilot implementation phase, PROMs were found to be feasible for use in primary care settings, particularly when workflows were adapted to local contexts with consideration for workload and the immediate practical benefits experienced by providers. PROMs were especially effective as a communication-opening tool that increased patient awareness of the importance of self-management and helped identify important symptoms within a short period of time. Implementation outcomes reflected satisfactory levels of acceptability, feasibility, adoption, appropriateness, and compatibility with existing service workflows. However, the limited implementation period was insufficient to capture long-term health outcomes. The findings suggest that PROMs can serve as an important tool for integrating patient-reported information into service-related decision-making and creating new experiences for both healthcare providers and patients. This demonstrates the potential of PROMs to support the development of Value-Based Health Care (VBHC). Future scale-up efforts should emphasize flexible system design, workforce capacity building, and long-term outcome monitoring.
