บทคัดย่อ
การบรรลุเป้าหมายการกำจัดโรคหัดในประเทศไทยจำเป็นต้องฟื้นฟูระดับภูมิคุ้มกันของประชากรให้สูงเพียงพอที่จะตัดวงจรการแพร่เชื้อได้ การระบาดที่เกิดขึ้นในช่วงหลังสะท้อนให้เห็นถึงช่องว่างของภูมิคุ้มกันที่ขยายตัว แม้ว่าประเทศไทยจะมีโครงการสร้างเสริมภูมิคุ้มกันโรคแห่งชาติที่ดำเนินมาอย่างต่อเนื่องยาวนานก็ตาม เราประเมินภูมิคุ้มกันจำเพาะต่อโรคหัดโดยใช้ตัวอย่างซีรัมที่เป็นตัวแทนตามช่วงอายุจำนวน 3,675 ตัวอย่าง ซึ่งเก็บในปี พ.ศ. 2567 จาก 4 จังหวัด วัดความเข้มข้นของ IgG ด้วยวิธี ELISA โดยกำหนดค่าระดับป้องกันที่ ≥200 IU/L และเปรียบเทียบกับการสำรวจซีรั่มระดับประเทศในปี พ.ศ. 2547 และ พ.ศ. 2557 เพื่อประเมินการเปลี่ยนแปลงของภูมิคุ้มกันตามช่วงเวลาในกลุ่มรุ่นปีเกิดภายใต้ยุควัคซีน ผลการศึกษาพบว่า ความชุกของผู้มีภูมิคุ้มกันในระดับป้องกันโดยรวมในปี พ.ศ. 2567 อยู่ที่ 61.3% และมีรูปแบบตามอายุเป็นลักษณะโค้งรูปตัว U อย่างเด่นชัด ผู้ใหญ่อายุมากกว่า 40 ปีมีระดับภูมิคุ้มกันป้องกันสูงกว่า 95% ซึ่งสอดคล้องกับการได้รับภูมิคุ้มกันจากการติดเชื้อตามธรรมชาติ ในทางตรงกันข้าม วัยรุ่นและวัยผู้ใหญ่ตอนต้นอายุ 14-31 ปี มีระดับภูมิคุ้มกันต่ำกว่ามาก อยู่ในช่วง 27-46% การวิเคราะห์ตามรุ่นปีเกิดพบการลดลงอย่างต่อเนื่องของสัดส่วนผู้มีภูมิคุ้มกันในกลุ่มผู้ที่เกิดระหว่างปี พ.ศ. 2528 - 2551 ซึ่งบ่งชี้ถึงการลดลงของภูมิคุ้มกันที่เกิดจากวัคซีนตามกาลเวลา ข้อมูลผู้ป่วยโรคหัดระดับประเทศในปี พ.ศ. 2567 สะท้อนผลในทิศทางเดียวกัน โดยผู้ติดเชื้อส่วนใหญ่นอกพื้นที่จังหวัดชายแดนภาคใต้พบในผู้มีอายุมากกว่า 15 ปี ระดับภูมิคุ้มกันของประชากรไทยในปัจจุบันยังต่ำกว่าค่าระดับภูมิคุ้มกันหมู่ที่จำเป็นต่อการหยุดยั้งการแพร่เชื้อโรคหัด ซึ่งประเมินว่าต้องอยู่ที่ 92% ขึ้นไป ความไวต่อการติดเชื้อที่สูงในวัยรุ่นและผู้ใหญ่บ่งชี้ว่า การได้รับวัคซีนตามกำหนดปกติ 2 เข็มอาจไม่เพียงพอภายใต้บริบททางระบาดวิทยาปัจจุบัน ผลการศึกษานี้สนับสนุนการพิจารณาให้วัคซีนป้องกันโรคหัดเข็มที่ 3 ในประชากรอายุ 10-40 ปี การบูรณาการวัคซีนเข็มดังกล่าวเข้ากับโปรแกรมวัคซีนกระตุ้นในวัยรุ่นหรือผู้ใหญ่ ร่วมกับการเสริมความเข้มแข็งของระบบเฝ้าระวังและการให้วัคซีนเก็บตกแบบมุ่งเป้าในพื้นที่ที่มีความครอบคลุมต่ำ อาจเป็นแนวทางเชิงปฏิบัติที่ช่วยปิดช่องว่างของภูมิคุ้มกันและเร่งความก้าวหน้าสู่การกำจัดโรคหัด
บทคัดย่อ
Achieving measles elimination in Thailand requires restoring population immunity to
levels sufficient to interrupt transmission. Recent outbreaks indicate widening immunity gaps
despite long-standing national immunization programs. We assessed measles-specific immunity
using 3,675 age-representative serum samples collected in 2024 from four provinces. IgG
concentrations were measured by ELISA with a protective threshold of ≥200 IU/L and
interpreted alongside national serosurveys conducted in 2004 and 2014 to evaluate temporal
changes in immunity across vaccine-era birth cohorts. Overall seropositivity in 2024 was 61.3
percent and showed a pronounced U-shaped age pattern. Adults older than 40 years had high
seroprotection above 95 percent consistent with natural infection. Adolescents and young
adults aged 14 to 31 years had much lower immunity ranging from 27% to 46%. Birth-cohort
analysis revealed progressive declines in seroprotection among individuals born between 1985
and 2008, indicating low levels of vaccine-induced immunity. National case data from 2024
reflected these findings, with most infections outside deep southern provinces occurring in
individuals older than 15 years. Thailand’s current immunity levels fall below the herd
immunity threshold needed to halt measles transmission, estimated at 92 percent or higher.
The high susceptibility in adolescents and adults suggests that two routine doses are
inadequate under current epidemiologic conditions. These results suggest the findings warrant
further evaluation of targeted booster strategies, including potential role of a third measlescontaining vaccine dose for individuals aged 10 to 40 years. Integrating this dose into adolescent
or adult booster programs, along with strengthened surveillance and targeted catch-up
vaccination in low-coverage regions, offers a practical pathway to closing immunity gaps and
accelerating progress toward measles elimination.
