บทคัดย่อ
ปัจจุบัน สภาเภสัชกรรมได้ออกประกาศ เรื่องการกำหนดมาตรฐานและขั้นตอนการให้บริการเภสัชกรรมทางไกล อย่างไรก็ตาม ในการดำเนินงานจำเป็นต้องมีรายละเอียดเพิ่มเติม เพื่อให้มีแนวทางการดำเนินงานเภสัชกรรมทางไกลที่เป็นไปอย่างต่อเนื่อง มีประสิทธิภาพ สอดคล้องกับการเปลี่ยนแปลงของสังคมและเทคโนโลยีในปัจจุบัน การศึกษานี้มีวัตถุประสงค์เพื่อจัดทำแนวทางขั้นพื้นฐานของระบบบริการเภสัชกรรมทางไกลและแนวทางการบริบาลเภสัชกรรม รวมทั้งจัดทำสถาปัตยกรรมข้อมูล (Data Architecture) ที่รองรับการดำเนินงานเภสัชกรรมทางไกล การศึกษานี้เป็นโครงการเชิงพัฒนา การดำเนินการวิจัย แบ่งเป็น 2 ระยะ ระยะที่ 1 เริ่มจากการทบทวนวรรณกรรม จาก guideline/standard of practice ของต่างประเทศ และการจัดการขนส่งซึ่งเป็นมาตรฐานที่ได้รับการยอมรับ วิเคราะห์ความเหมาะสมของมาตรฐานต่างประเทศกับบริบทของประเทศไทย ; ทีมวิจัยสังเคราะห์ร่างแนวทางขั้นพื้นฐานของระบบบริการเภสัชกรรมทางไกล แนวทางการบริบาลเภสัชกรรม ; สนทนากลุ่มผู้มีส่วนเกี่ยวข้องและรับฟังความเห็นต่อร่างฯ จากผู้ประกอบวิชาชีพเภสัชกรรมและสรุปร่างแนวทางขั้นพื้นฐานของระบบบริการเภสัชกรรมทางไกล แนวทางการบริบาลเภสัชกรรม การดำเนินงานในระยะที่ 2 เป็นการจัดทำร่างสถาปัตยกรรมข้อมูลที่รองรับการดำเนินงานเภสัชกรรมทางไกลโดยใช้ร่างแนวทางขั้นพื้นฐานของระบบฯ ที่เป็นข้อสรุปจากระยะที่ 1 หลังจากนั้นจัดรับฟังความเห็นในกลุ่มผู้ที่เกี่ยวข้องและสรุปสถาปัตยกรรมข้อมูลที่รองรับการดำเนินงานเภสัชกรรมทางไกล ผลการศึกษาระยะที่ 1 ในการจัดทำแนวทางขั้นพื้นฐานของระบบบริการเภสัชกรรมทางไกลและแนวทางการบริบาลเภสัชกรรม ได้แก่ ชุดข้อมูลพื้นฐานประกอบด้วย ข้อมูลผู้ป่วย ใบสั่งยา การจ่ายยา การบริบาลเภสัชกรรมและการขนส่งยา แนวทางขั้นพื้นฐานของระบบบริการเภสัชกรรมทางไกล ประกอบด้วย 8 องค์ประกอบ แนวทางการบริบาลเภสัชกรรม ได้แก่ การวิเคราะห์ใบสั่งยาและจัดเตรียมยาตามใบสั่งยาทางไกล (remote prescription) การขนส่งยา (drug transportation) การให้คำปรึกษาด้านยาทางไกล (remote counseling) ผลการศึกษาระยะที่ 2 ได้แก่ (ร่าง) Data Architecture สำหรับส่งข้อมูลโดยใช้ HL7-FHIR โดยกำหนดมาตรฐานข้อมูล ซึ่งจะนำไปสู่ความสามารถในการแลกเปลี่ยนข้อมูล ที่จะทำให้เกิดการทำงานร่วมกันระหว่างหน่วยงาน บนพื้นฐานข้อมูลชุดเดียวกัน ส่งผลต่อผลลัพธ์ในการดูแลผู้ป่วยที่ดีต่อไป ข้อเสนอแนะจากการศึกษาครั้งนี้ ประกอบด้วย ข้อเสนอต่อสภาเภสัชกรรมในการประสานงานเพื่อให้เกิดการประกาศใช้แนวทางการบริการเภสัชกรรมทางไกล ข้อมูลมาตรฐานและข้อเสนอในการพัฒนาศักยภาพของเภสัชกรชุมชนให้รองรับระบบบริการเภสัชกรรมทางไกล
บทคัดย่อ
At present, The Pharmacy council has launched the notification on Guidelines for Telepharmacy, with some additional details required to be stated for the setting of continuous and efficient operating in telepharmacy, while conforming current changes of society and technology. This study aimed to conduct the standard of telepharmacy and guidelines for pharmaceutical care and to develop data architecture which supports and facilitates the operating of telepharmacy. This study was designed in accordance with development research, in which it was divided into 2 phases. The first phase, literature review was done from guideline/standard of practice of foreign references and acceptable standard transportation requirements which was primarily analyzed for the suitability in consistency of context. Team on synthesis research has drafted the guideline of telepharmacy and guidelines for pharmaceutical care. Summary of the guidelines was obtained according to the group discussions of stakeholders and opinions from pharmacists. In the second phase, the draft of data architecture for supporting the telepharmacy was conducted in accordance with information from phase I. Additional suggestions from stakeholders were obtained, prior to the summary of data architecture. Phase I study result provided the basic guideline for telepharmacy that was consisted of minimum dataset (composing of patients’ information, prescription, dispensing, pharmaceutical care service, and drug transportation); the basic guideline for telepharmacy (in which it contained 8 components i.e., location, customer service approach, information technology, transportation, etc.); and guidelines for pharmaceutical care (including prescription analysis and pre-dispensing for remote prescription, drug transportation, and remote counseling. The result of phase II study provided the draft of data architecture using HL7-FHIR for standard messaging and standardizing the database which contributes to the data exchange among organizations and leads further to better outcome in patient care. Finally, this study proposed the Pharmacy Council of Thailand to implement the guidelines, standard data set, and capacity building for community pharmacist on telepharmacy service.
