dc.description.abstractประเทศไทยเริ่มให้วัคซีนป้องกันไวรัสตับอักเสบ บี แก่เด็กแรกเกิดทั่วทั้งประเทศในปี พ.ศ. 2535 และต่อมาก็มีการปรับเปลี่ยนตารางการให้วัคซีนแก่เด็กและในอนาคตองค์การอนามัยโลกจะยกเลิกการให้วัคซีนโปลีโอแบบหยอดเป็นแบบฉีด โดยเป็นวัคซีนรวมป้องกัน 5 หรือ 6 โรค โดยขึ้นอยู่กับวัคซีนไอกรนที่ใช้อยู่ก็มี 2 ชนิด คือ whole cell และ acellular ตามลำดับ ในการศึกษานี้จึงเป็นการให้วัคซีนรวมป้องกัน 5 หรือ 6 โรค แก่เด็กอายุแรกเกิด 2, 4, 6 และ 18 เดือน เพื่อตรวจหาระดับภูมิต้านทานไวรัสตับอักเสบ บี (anti-HBs) มีระดับเพียงพอต่อกันป้องกันโรคในระยะยาว เมื่อทำการตรวจหาระดับ anti-HBs ในเด็กแรกเกิดหรือก่อนได้รับวัคซีน ช่วงอายุ 7, 18, 19, 24, 36, 48, 60 และ 72 เดือน พบว่า กลุ่มเด็กที่ได้รับวัคซีนชนิด acellular นั้นมีระดับ anti-HBs ที่สูงกว่า 1 log scale เมื่อเทียบกับกลุ่มเด็กที่ได้รับวัคซีนรวมชนิด whole cell แต่การให้วัคซีนรวมทั้งสองชนิดต่อเด็กอายุ 18 เดือน หรือวัคซีนเข็มที่ห้านั้น พบว่า เด็กที่อายุ 19 เดือน มีระดับ anti-HBs นั้นสูงกว่าภายหลังจากการได้รับวัคซีนเข็มที่สี่ คาดว่าการได้รับวัคซีนป้องกันไวรัสตับอักเสบ บี อย่างน้อยห้าเข็มนั้นอาจจะส่งผลให้ระดับ anti-HBs เพียงพอต่อการป้องกันการติดเชื้อในระยะยาวได้มากกว่าการให้วัคซีนตามแบบแผนปัจจุบันที่ให้อย่างน้อยสี่เข็ม ถึงแม้ระดับ anti-HBs และค่าเฉลี่ย anti-HBs หรือค่า GMT จะเริ่มลดลงตั้งแต่อายุ 2 ปี และลดลงอย่างเห็นได้ชัดตั้งแต่อายุ 3 ปี แต่เมื่ออาสาสมัครอายุถึง 6 ปี ค่า GMT ของอาสาสมัครก็ยังคงอยู่ log scale เดียวกัน และจากผลการศึกษาแสดงให้เห็นว่าในเด็กอายุ 5 ปี มีระดับเพียงพอต่อการป้องกันโรค หรือ anti-HBs ≥ 10 mIU/ml ร้อยละ 94.2 และ 62 จากการได้รับวัคซีนรวมชนิด acellular และ whole cell ตามลำดับ จึงเป็นไปได้ว่าการได้รับวัคซีนป้องกันไวรัสตับอักเสบ บี อย่างน้อยห้าเข็มนั้นน่าจะส่งผลให้ระดับ anti-HBs เพียงพอต่อการป้องกันการติดเชื้อในระยะยาวได้เช่นเดียวกับการศึกษาการให้วัคซีนป้องกันไวรัสตับอักเสบ บี ชนิดเดี่ยว ที่จะอยู่ยาวนานมากกว่า 20 ปี นอกจากนี้ผลการศึกษาการตอบสนองของภูมิต้านทานต่อไวรัสตับอักเสบ บี หลังให้วัคซีนไวรัสตับอักเสบ บี เมื่อแรกเกิดและวัคซีนรวมป้องกัน 5 โรค ที่ 2, 4, 6 เดือน ในแผนการให้วัคซีนแห่งชาติ เมื่อเดือนมิถุนายน พ.ศ. 2562 พบว่า มีระดับภูมิคุ้นกันต่อไวรัสตับอักเสบ บี ที่สูง ทั้งภายหลังการฉีดเข็มสุดท้ายไปแล้ว 1 เดือน และ 6 เดือน ซึ่งผลไม่ต่างกับวัคซีนรวมป้องกันหกโรคจากการศึกษาก่อนหน้านี้ ดังนั้น วัคซีนรวมป้องกันห้าโรคในแผนการให้วัคซีนแห่งชาตินี้ มีระดับเพียงพอต่อการป้องกันโรคในระยะยาวได้เป็นอย่างดียิ่งth_TH

dc.description.abstractalternativeThe universal hepatitis B vaccination for Thai newborns was integrated into the EPI in 1992. After that HB vaccine schedule had to be changed for facility to provide the vaccination. In the future, WHO will be not provide a weakened poliovirus given by mouth to infants and changed to be an inactivated poliovirus given by injection as combine vaccine likes pentavalent (wP) or hexavalent (aP) vaccine which result in the infant will be received five doses of HB vaccination in age at birth, 2, 4, 6 and 18 months. Therefore, this study is the long term to determine anti-HBs titers in the new born whom were received combine wP or aP vaccine.This study is determine anti-HBs titer in the new borns in age at birth, 7, 18, 19, 24, 36, 48, 60 and 72 months. And the results show that aP vaccine group had the anti-HBs titer and GMT level higher than wP vaccine group. However, the vaccination in age 18 months (5th dose) was influent to stimulate anti-HBs titers more than 4th dose for the both of combine vaccine. Then the long-term study show that the ant-HBs titer was initiate decreasing at age 2 year old and rapidly dropping at age 3 year old but the GMT level was remain in the same log scale in age between 3 - 6 year old. Children aged 6 year old had the level of anti-HBs (anti-HBs ≥ 10 mIU/ml) 94.2 and 62 of aP and wP vaccine group, respectively. Thus, the 5th dose for the both of combine vaccine was influent to stimulate the long term protective HB infection likes the long term study of the antibodies persistence more than 20 years after infant received monovalenct hepatitis B vaccination. Moreover, the study of antibody response to hepatitis B after immunization with monovalent hepatitis B vaccine at birth, and DTPw-HiB-HB+OPV at 2, 4, 6 months as part of national program immunization was imitating in June, 2019. After 1 and 6 moth of the 4th dose stimulation found that anti-HBs and GMT level was highly increasing levels and similar with the results of aP vaccine group at age 7 and 18 month. Therefore, the combine vaccine in EPI program might be highly effective long term against HBV infection.th_TH
